摘要
目的探讨小切口腔镜辅助甲状腺切除术对分化型甲状腺癌患者术中出血量及并发症的影响。方法选择2016年9月-2018年9月间于洛阳市第一人民医院接受治疗的分化型甲状腺癌患者72例,依照随机数字表法分为观察组(36例)和对照组(36例)。对照组采用传统开放手术治疗,观察组采用小切口腔镜辅助甲状腺切除术治疗,将两组手术相关指标(手术时间、术中出血量、切口长度、拆线时间、住院时间)与术后并发症(切口感染、继发性出血、低钙血症)进行比较。结果观察组手术时间[(108.68±20.78)min]长于对照组[(80.34±16.02)min],拆线时间、住院时间[(4.39±0.56)d、(5.24±0.63)d]短于对照组[(6.24±0.63)d、(7.64±1.03)d],术中出血量[(12.42±2.58)ml]少于对照组[(30.42±2.12)ml],切口长度[(2.21±0.43)cm]短于对照组[(7.21±2.65)cm],差异有统计学意义(P<0.05);观察组术后并发症发生率(8.33%)低于对照组(30.56%),差异有统计学意义(P<0.05)。结论小切口腔镜辅助甲状腺切除术治疗分化型甲状腺癌患者,具有手术创伤小、术后恢复快与术后并发症少的优点,但手术时间较长,医院应根据自身医疗资源与患者个人情况酌情选择。
Objective To study the effect of small-cut oral-mirror-assisted thyroidectomy on the amount of blood loss and complications in the operation of differentiated thyroid carcinoma.Methods 72 cases of differentiated thyroid carcinoma treated in the hospital from September,2016 to September,2018 were divided into the observation group(36 cases)and the control group(36 cases)according to the random number table method.The control group was treated with traditional open surgery,and the observation group was treated with small-cut oral-mirror-assisted thyroidectomy.Operation-related indexes(operation time,intraoperative blood loss,incision length,time of break-off,hospital stay)and postoperative complications(incision infection,follow-up paroxysmal hemorrhage,hypocalcemia)were compared.Results The operation time[(108.68±20.78)min]in the observation group was longer than that in the control group[(80.34±16.02)min].The time of disconnection and hospitalization[(4.39±0.56)d,(5.24±0.63)d]were shorter than those in the control group[(6.24±0.63)d,(7.64±1.03)d],and the amount of intraoperative bleeding[(12.42±2.58)ml]was less than that in the control group[(30.42±2.12)ml].Incision length of the observation group[(2.21±0.43)cm]was shorter than that in the control group[(7.21±2.65)cm].The difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group(8.33%)was lower than that in the control group(30.56%),and the difference was statistically significant(P<0.05).Conclusion The treatment of patients with differentiated thyroid cancer with small incision and assisted thyroidectomy has the advantages of small surgical trauma,rapid recovery and less complications,but the operation time is longer.Hospitals should select the treatment method according to medical resource and patients’situation.
