摘要
目的探讨高危型人乳头状瘤病毒(HR-HPV)分型检测联合薄层液基细胞学检查(TCT)在宫颈病变筛查中的应用价值。方法选取2017年3月~2018年3月我院收治的宫颈病变患者416例为研究对象,均行HR-HPV分型检测、TCT检查以及阴道镜病理活检,以组织病理学检查结果为“金标准”,比较HR-HPV分型检测与TCT检查对宫颈癌的诊断价值。结果416例患者中病理诊断为炎症的213例,CINⅠ102例、CINⅡ69例、CINⅢ25例,鳞状细胞癌7例。联合检测的敏感性、特异性、诊断符合率分别98.52%、100.00%、99.28%分别高于TCT检查的79.80%、84.51%、82.21%以及HR-HPV检测的89.66%、63.38%、76.20%,差异有统计学意义(P<0.05)。结论HR-HPV分型联合TCT检测能够提高宫颈病变的检出率,降低漏诊率,对宫颈癌的防治具有重要意义。
Objective To investigate the application value of high-risk human papilloma virus(HR-HPV)typing and thin-layer liquid-based cytology(TCT)in screening for cervical lesions.Methods A total of 416 patients with cervical lesions admitted in our hospital from March 2017 to March 2018 were selected as the research subjects.All patients underwent HR-HPV typing,TCT and colposcopy pathological biopsy.The results of histopathological examination were"gold standard"to compare the diagnostic value of HR-HPV typing test and TCT test for cervical cancer.Results Among 416 patients,213 were pathologically diagnosed as inflammation,102 were CINⅠ,69 were CINⅡ,25 were CINⅢ,and 7 were squamous cell carcinoma.The sensitivity,specificity,and diagnostic coincidence rates of the combined tests were 98.52%,100.00%,and 99.28%,respectively,which was higher than 79.80%,84.51%,82.21%of TCT test and 89.66%,63.38%,76.20%of HR-HPV test,the difference was statistically significant(P<0.05).Conclusion HR-HPV classification combined with TCT detection can improve the detection rate of cervical lesions and reduce the rate of missed diagnosis,which is of great significance for the prevention and treatment of cervical cancer.
