经皮椎体后凸成形术治疗不同BMI骨质疏松性椎体压缩性骨折患者的临床疗效 被引量:29

Clinical efficacy of percutaneous kyphoplasty in the treatment of osteoporotic vertebral compression fracture patients with different BMIs

摘要 目的观察经皮椎体后凸成形术(PKP)治疗不同BMI骨质疏松性椎体压缩性骨折(OVCF)患者的临床疗效。方法根据不同BMI,86例行PKP治疗的OVCF患者分为A组(BMI<18.5 kg/m^2,29例)、B组(BMI 18.5~23.9 kg/m^2,31例)和C组(BMI>23.9 kg/m^2,26例)。比较三组手术相关指标及并发症发生情况。结果三组手术时间、术中出血量、住院时间、骨水泥注入量及骨水泥渗漏率差异均无统计学意义(P>0.05)。与术前相比,三组术后2 d及末次随访时的VAS疼痛评分、Oswestry功能障碍指数评分、椎体前缘高度和椎体后凸Cobb角均改善(P<0.05)。B组和C组末次随访时的椎体前缘高度和椎体后凸Cobb角恢复优于A组(P<0.05)。结论 PKP治疗OVCF的疗效显著,能快速缓解疼痛,短期内改善患者术后生活质量。与正常和高BMI患者相比,低BMI患者再骨折发生率较高,术后椎体高度改善较差,故BMI可作为影响OVCF患者PKP预后的一项重要指标。 Objective To observe the clinical efficacy of percutaneous kyphoplasty(PKP) in the treatment of osteoporotic vertebral compression fracture(OVCF) patients with different BMIs.Methods On the basis of different BMIs,86 OVCF patients were divided into three groups of A(BMI<18.5 kg/m2,29 cases),B(BMI 18.5-23.9 kg/m2,31 cases) and C(BMI>23.9 kg/m2,26 cases).The surgery-related indicators and complications were compared among three groups.Results There were no statistical differences in the operation time,intraoperative blood loss,hospital stay,amount of bone cement injection and rate of bone cement leakage among three groups(P>0.05).Compared with before,VAS pain score, Oswestry disability index score,height of anterior vertebral body and Cobb’s angle were significantly improved on the 2nd day after operation and at the latest follow-up in three groups(P<0.05).The height of anterior vertebral body and Cobb’s angle in groups of B and C were better than those in group A at the latest follow-up(P<0.05).Conclusion PKP has obviously curative effect on OVCF,and can quickly relieve pain and improve life quality of patients in a short time.Compared with the patients with normal or higher BMI,the incidence of refracture is increased and height of vertebral body is poor in the patients with lower BMI.Therefore,BMI could be an important indicator influencing the prognosis of OVCF patients underwent PKP.
