摘要
目的探讨应用"居家糖尿病护理平台"对早发2型糖尿病患者进行延续护理的效果。方法选择176例早发2型糖尿病患者,应用"居家糖尿病护理平台"进行延续护理,连续随访6个月,比较患者管理前后自我管理能力和代谢指标。结果早发2型糖尿病患者应用"居家糖尿病护理平台"干预后患者饮食、运动、血糖监测等6项自我管理行为评分高于干预前,糖化血红蛋白、收缩压、舒张压、胆固醇、低密度脂蛋白胆固醇、甘油三酯等与干预前比较差异均有统计学意义(P<0.01);高密度脂蛋白胆固醇、体质指数与干预前比较,差异无统计学意义(P>0.5)。结论应用"居家糖尿病护理平台"进行延续护理提高了早发2型糖尿病患者自我管理能力,改善了代谢指标。
Objective To explore the effect of " home diabetes care platform" in continuous nursing of patients with early-onset type2 diabetes.Methods A total of 176 patients with early-onset type 2 diabetes were selected to receive continuous nursing from " home diabetes care platform".The patients were followed up for 6 months and their self-management behavior and metabolic markers were compared before and after internation.Results After application of " home diabetes care platform",the scores of 6 self-management behaviors such as patients’ diet,exercise,blood glucose test were higher than those before intervention.Hemoglobin,systolic blood pressure,diastolic blood pressure,cholesterol,low density lipoprotein cholesterol and triglyceride had significantly statistical differences(P<0.01).However,there was no significant differences in high-density lipoprotein cholesterol,body mass index(P>0.05).Conclusion The application of continuous care based on "home diabetes care platform" has significant effect on improving self-management behavior and metabolic indexes of early-onset type 2 diabetes patients.
