摘要
目的分析糖尿病前期患者授权能力、自我效能与自我管理之间的内在关系,探索患者自我管理行为的形成机制,为护理人员设计基于授权理论的糖尿病干预方案提供理论支撑。方法选取上海市松江区车墩镇社区卫生服务中心200例糖尿病前期患者作为研究对象,于2018年采用糖尿病授权评分量表、糖尿病管理自我效能量表、糖尿病自我管理量表对其进行问卷调查,采用结构方程模型建立三个变量之间的内在结构关系。数据统计在SPSS 25.0和AMOS24.0中进行,以P<0.05为具有统计学意义。结果授权能力和自我效能,均与自我管理之间存在显著正相关关系(r>0,P<0.05)。授权能力在三者中起中介作用:未加入授权能力这一变量时,自我效能和自我管理之间关系显著(P<0.05);加入授权能力这一变量后,关系不再显著(P>0.05)。结论糖尿病前期患者的授权能力是自我效能影响自我管理行为的内在机制。护理人员应结合周围支持资源,设计基于授权理论的糖尿病干预方案,通过提升糖尿病前期患者的自我授权能力,加快患者从有信心开展(自我效能)到实际开展(自我管理)健康促进项目的进程,有效防止和降低糖尿病前期患者转归为糖尿病患者,最终提高糖尿病前期患者的远期生活质量。
Objective This study analyses the relationship among empowerment,self-efficacy and self-management of patients with pre diabetes to explore the formation mechanism of patients’self-management behavior and provide theoretical basis for nurses to design intervention program based on empowerment theory.Methods 200 cases of prediabetes in Chedun community health service center in Songjiang area of Shanghai were selected as the research objects.They were investigated with diabetes empowerment scale,diabetes management self-efficacy scale and diabetes self-management scale in2018.The structural equation model was used to build the internal structural relationship among the three variables.Data statistics were conducted in SPSS24.0 and AMOS24.0,with P<0.05 as statistically significant.Results There is a significant positive correlation between empowerment and self-management,so is self-efficacy and self-management(r>0,P<0.05).Empowerment plays a mediating role in the three,the relationship between self-efficacy and self-management is significant(P<0.05)when the variable of empowerment was not added,and the relationship was no longer significant(P>0.05)otherwise.Conclusion Empowerment ability of patients with pre diabetes mellitus is the internal mechanism of self-efficacy influencing self-management behavior.Nursing staff should combine with supportive resources to design intervention program based on empowerment theory.By improving self-empowerment ability of patients with pre diabetes,patients can accelerate the process from being confident to carry out(self-efficacy)to actually carrying out(self-management)health promotion projects,which is effective to prevent and reduce the conversion of patients with pre diabetes to diabetic patients,and ultimately improve the long-term quality of life of patients with pre diabetes.
