摘要
目的探讨原发性高血压患者合并心房颤动的危险因素。方法回顾性分析我院诊治的256例原发性高血压患者临床资料,根据是否合并心房颤动分为观察组87例(合并心房颤动者)和对照组169例(未合并心房颤动者)。对两组一般临床资料、血液生化指标、心脏彩超参数进行单因素分析及多因素非条件logistic回归分析寻找原发性高血压合并心房颤动的危险因素。结果经多因素非条件logistic回归分析显示,高龄、伴房性心动过速、低密度脂蛋白胆固醇(LDL-C)偏低、N末端B型脑利钠肽前体(NT-proBNP)偏高、左房内径(LAD)偏大及左室后壁厚度(LVPWT)偏大是原发性高血压患者合并心房颤动的危险因素。结论原发性高血压合并心房颤动与年龄、房性心动过速、LDL-C、NT-proBNP、LAD、LVPWT密切相关,临床应对上述指标检出异常的原发性高血压患者引起高度重视。
Objective To explore the influencing factors of atrial fibrillation in patients with essential hypertension.Methods The clinical data of 256 patients with essential hypertension in our hospital were retrospectively analyzed.According to presence or absence of atrial fibrillation,they were divided into observation group(with atrial fibrillation,n=87)and control group(without atrial fibrillation,n=169).Univariate analysis and unconditional multivariate logistic regression analysis were performed on general clinical data,blood biochemical parameters and indicators of cardiac color Doppler between the two groups,to analyze the risk factors for essential hypertension with atrial fibrillation.Results Unconditional multivariate logistic regression analysis showed that advanced age,atrial tachycardia,low low-density lipoprotein cholesterol(LDL-C)level,high N-terminal pro-brain natriuretic peptide(NT-proBNP)level,large left atrial diameter(LAD)and large left ventricular posterior wall thickness(LVPWT)were risk factors for atrial fibrillation in patients with essential hypertension.Conclusion Essential hypertension with atrial fibrillation is closely related to age,atrial tachycardia,LDL-C,NT-proBNP,LAD and LVPWT.Therefore,it is necessary to pay attention to essential hypertensive patients with abnormal detection of the above indicators in clinical practice.
