高血压病患者发生心房颤动的相关危险因素临床分析 被引量:12

摘要 目的:探讨高血压病患者发生心房颤动(AF)的临床相关危险因素。方法:将423例高血压病患者分为高血压病合并AF(n=233),不合并AF(n=190)两组,回顾性分析比较两组患者的临床资料,并进行Logistic回归分析高血压病患者发生AF的可能危险因素。结果:单变量分析显示,与非AF组相比,AF组的年龄较大(P<0.001)、体重指数偏高(P=0.019),心脏超声测量左心房内径、左室舒张末期内径和左室收缩末期内径均偏大(P<0.001),左室射血分数(P=0.005)较低。肌酐(P<0.001)和尿酸水平(P=0.013)偏高,但总胆固醇及低密度脂蛋白胆固醇水平(P<0.001)较低。Logistic多因素回归分析提示,年龄(OR=1.08,95%CI:1.04~1.13,)、左心房内径(OR=1.51,95%CI:1.38~1.63)、低密度脂蛋白胆固醇含量(OR=0.52,95%CI:0.36~0.78)与高血压病患者发生AF存在显著的关联关系。结论:年龄及左心房内径越大、低密度脂蛋白胆固醇水平越低的高血压病患者发生AF的危险性高。 Objective:To investigate risk factors for the incidence of atrial fibrillation(AF) in hypertensive patients.Methods:423 patients with hypertension were enrolled in our study from January 2009 to December 2011.Subjects were divided into AF(233 cases) group or Non-AF group(190 cases).The clinical data were collected and Logistic regression analysis was performed.Results:Compared with Non-AF group,patients in AF group were older(P<0.001),with greater body mass index(BMI)(P=0.019),higher level of uric acid(P=0.013) and creatinine(P<0.001),greater echocardiography measurement of left atrial dimension,left ventricular end-diastolic dimension and left ventricular end-systolic dimension(P<0.001),but had lower left ventricular ejection fraction(P=0.005) and low-density lipoprotein cholesterol(LDL-C)(P<0.001).Multivariate logistic regression analysis indicated the independent risk factors for the incidence of AF in hypertensive patients were age(OR=1.08,95% CI: 1.04~1.13),left atrial dimension(OR=1.51,95% CI: 1.38~1.63) and LDL-C(OR=0.52,95% CI: 0.36~0.78).Conclusion:Age and left atrial dimension were independent risk factors for the onset of AF,while lower levels of LDL-C were associated with a higher incidence of AF in hypertensive patients.
