320排CT低剂量冠状动脉成像评价左心功能 被引量:5

Value of 320-row low-dose CT in left ventricular function analysis

摘要 目的探讨320排CTCA低剂量成像评价左心功能的价值。方法回顾性分析临床怀疑冠状动脉粥样硬化性心脏病(冠心病)行320排CTCA左心功能检查者100例,常规组与低剂量组各50例。其中男57例,女43例,平均59.30±12.29岁,分别记录两组图像质量、辐射剂量(DLP)、有效剂量(ED)。回顾性收集在一周内分别行320排CTCA和2D-TTE检查者94例,其中男46例,女48例,平均61.14±12.71岁,两种方法分别测量左心室舒张末期容积(EDV)、收缩末期容积(ESV)、射血分数(EF)、每搏输出量(SV)和每分输出量(CO)值。对正态分布的变量采用独立样本t检验或配对样本t检验,对非正态分布的变量采用Mann-Whitney U检验或Wilcoxon配对样本符号秩检验,并进行Pearson相关性分析。P<0.05认为有统计学差异。结果低剂量组在年龄、体质量指数(BMI)、图像质量方面与常规组比较无统计学差异,ED较常规组明显降低约36.09%(P=0.000),平均ED为4.94±2.24 mSv。CTCA测量的EDV、EF、SV值略高于2D-TTE。EDV、ESV、EF、SV与2D-TTE测量值呈中等程度显著性相关(r=0.420、0.732、0.581、0.516)。结论320排CTCA低剂量成像评价左心功能与2D-TTE一致性较好,值得在临床中推广应用。 Objective To explore the value of 320-row low dose CT in the assessment of left ventricular function.Methods 100 patients(57 men,43 women;mean age:59.30±12.29 years)with clinically suspected coronary heart disease underwent 320-row CT coronary angiogram(CTCA)including left heart function with 50 patients in the conventional group and 50 patients in the low-dose group.The image quality,dose-length product(DLP)and effective dose(ED)were recorded.CTCA and 2-D transthoracic echocardiogram(TTE)were performed within one week on 94 patients(46 men,48 women;mean age:61.14±12.71 years).The left ventricular end-diastolic volume(EDV),end-systolic volume(ESV),ejection fraction(EF),stroke volume(SV)and cardiac output(CO)using the two imaging methods were compared by independent-variable t-test or paired-sample t-test for variables with normal distribution,and Mann-Whitney U test or Wilcoxon paired sample sign rank test for variables with non-normal distribution.Pearson correlation analysis was performed.P<0.05 was considered statistically significant.Results The age,body mass index,and image quality of the low-dose CTCA group were not statistically different from those of the conventional CTCA group.The average ED of the low-dose group(4.94±2.24 mSv)was significantly reduced by 36.09%compared with the conventional group(P=0).The EDV,EF,and SV measured by CTCA were slightly higher than those of 2 D-TTE.The EDV,ESV,EF,and SV on CTCA showed moderately significant correlation(r=0.420,0.732,0.581,0.516)with that on 2 D-TTE.Conclusion 320-row low-dose CTCA has good consistency with 2D-TTE in evaluating left ventricular function.
