摘要
目的探讨早发冠心病患者家族性高胆固醇血症的冠脉造影及心功能特点。方法回顾性收集2012年6月至2017年6月在北京安贞医院经冠状动脉造影确诊为早发冠心病的患者856例(男<55岁,女<60岁),采用荷兰临床脂质网络(DLCN)标准在早发冠心病患者中进行家族性高胆固醇血症(FH)筛查,研究其发病率,观察FH患者的冠脉造影特征及其他临床特征。结果在该早发冠心病队列中,根据DLCN评分标准,37例患者(4.3%)为确定/可能性大的FH,256例患者(24.7%)为可能的FH,569例患者为FH排除组(65.8%)。FH与非FH患者相比,多支冠脉病变在FH患者中更为常见(P<0.001),FH患者的Gensini评分较高(P<0.01),FH患者的左室射血分数(LVEF)低于非FH组(P<0.01)。结论 FH是早发冠心病的独立危险因素,在早发冠心病中常见,早发冠心病患者中FH患者冠脉病变更为严重,心功能更差。
Objective To investigate the characteristics of coronary angiography and cardiac function of familial hypercholesterolemia in patients with early onset coronary heart disease.Methods We retrospectively collected 856 patients(male<55 years old,female<60 years old)with premature coronary heart disease diagnosed by coronary angiography in Beijing Anzhen Hospital from June,2012 to June,2017.Familial hypercholesterolemia(FH)screening was carried out in patients with premature coronary heart disease using Dutch clinical lipid network(DLCN)standard to study the incidence.Coronary angiographic features and other clinical features of FH patients were also observed.Results According to DLCN scoring criteria,37 patients(4.3%)were identified with a high probability of FH,256 patients(24.7%)were likely to develop FH,and 569 patients(65.8%)were in the exclusion group.Compared with non-FH patients,multi-vessel lesions were more common in FH patients(P<0.001).Gensini score was relatively higher in FH patients than non-FH patients(P<0.01).Left ventricular ejection fraction(LVEF)in FH patients was lower than that in non-FH patients(P<0.01).Conclusion FH is an common and independent risk factor for early-onset coronary heart disease.In patients with early-onset coronary heart disease,FH patients have more severe condition and worse heart function.
