摘要
目的探讨案例式教学模式(CBL)在医学遗传科住院医师规范化培训的应用。方法选取2017年12月-2018年12月在南京医科大学附属第一医院生殖医学科进行规范化培训的2017和2018级住院医师规范化培训学员共26名作为研究对象,随机分为观察组与对照组,每组13名。两组学员均由医学遗传科的带教老师进行教学查房和临床操作的培训,观察组以案例为基础教学模式进行培训,每周增加1~2次以案例为基础的临床思维能力培训课。以理论考核及操作技能考核成绩进行对比分析,并以调查表的形式进行教学成果调查。结果观察组的出科考核成绩中选择题得分(18.15±1.95)分,问答题得分(25.31±3.20)分,操作能力考核得分(87.08±4.97)分,对照组各项得分分别为(17.00±1.78)分、(24.46±3.18)分、(85.85±5.24)分,两组间相比差异无统计学意义(P> 0.05)。观察组病例分析题得分(42.77±3.13)分,明显高于对照组(29.92±5.94)(P <0.05)。教学效果评价问卷调查结果提示观察组临床思维、学习积极性、自学能力、解决问题的能力均提高,对教学过程更加满意,对疾病认知度更好。结论案例式教学模式对医学遗传科住院医师培训中具有很好的效果,值得推广应用。
Objective To explore the application of case-based teaching mode(CBL) in the standardized training of resident physicians in medical genetic. Method A total of 26 genetic residents for standardized training were selected from the Department of Reproductive Medicine of the First Affiliated Hospital of Nanjing Medical University from December 2017 to December 2018,who were randomly divided into observation group and control group, 13 in each group. Both groups were taught by the Department of Medical Genetics to conduct teaching rounds and clinical operation training, while the observation group used case-based teaching, weekly clinical thinking ability training courses increased for featuring case teaching. The theoretical assessment and operational skills assessment results were compared and analyzed, and the teaching results were investigated in the form of questionnaires. Result The average score of multiple-choice questions, questions and answers, operation ability assessment in the observation group and control group were not statistically significant(P>0.05), [(18.15±1.95) vs(17.00±1.78),(25.31±3.20) vs(24.46±3.18),(87.08±4.97) vs(85.85±5.24), respectively]. The scores of case analysis questions in the observation group(42.77±3.13) were significantly higher than those in the control group(29.92±5.94)(P<0.05).The results of the teaching effect evaluation questionnaire suggest that the observation group’s clinical thinking, learning enthusiasm, self-learning ability, and problem-solving ability are all improved, the teaching process is more satisfied, and the disease awareness is better. Conclusion The casebased teaching mode has a good effect on the resident training of medical genetics, and it is worthy of popularization and application.
