摘要
目的:研究活血利水法治疗Pilon骨折手术后患肢肿胀的临床效果。方法:选取2016年1月-2019年6月于本院住院治疗的Pilon骨折患者60例。按照随机数字表法将其分为治疗组和对照组,各30例。对照组采用静滴甘露醇治疗,治疗组在对照组基础上加用活血利水法治疗。观察两组术后患侧小腿肿胀值、治疗前后疼痛(VAS评分)及踝关节活动度(AOFAS评分)、并发症发生情况、临床疗效。结果:术后第1天,两组患侧小腿肿胀值比较,差异无统计学意义(P>0.05);术后第3天,治疗组患侧小腿肿胀值均低于对照组及术后第1天(P<0.05),但对照组术后第1、3天患侧小腿肿胀值比较,差异无统计学意义(P>0.05);术后第5、7天,两组患侧小腿肿胀值均低于术后第1天,且治疗组均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组VAS均低于治疗前,AOFAS评分均高于治疗前,且治疗组VAS评分低于对照组,AOFAS评分高于对照组,差异均有统计学意义(P<0.05)。治疗组并发症发生率6.67%,低于对照组的36.67%(P<0.05)。治疗组总有效率93.33%,高于对照组的66.67%,差异有统计学意义(字2=6.667,P=0.010)。结论:活血利水法治疗Pilon骨折手术后患肢肿胀是临床上较好的治疗方法,能有效降低小腿肿胀值、VAS评分和并发症发生率,提高踝关节活动度评分和临床疗效。
Objective:To study the clinical efficacy of Huoxue Lishui method in the treatment of limb swelling after Pilon fracture surgery.Method:From January 2016 to June 2019,60 patients with Pilon fracture who were hospitalized in our hospital were selected.According to the method of random number table,they were divided into treatment group and control group,30 cases in each group.The control group was treated with intravenous Mannitol,and the treatment group was treated with Huoxue Lishui method on the basis of the control group.The swelling of the affected leg,pain(VAS)and ankle joint activity(AOFAS)before and after treatment,complications and clinical effect were observed.Result:On the first day after operation,there was no significant difference between the two groups in swelling value of the affected leg(P>0.05).On the third day after operation,the swelling value of the affected leg in the treatment group were lower than those in the control group and the first day after operation(P<0.05),but the swelling value of affected leg in the control group was compared on the 1st and 3rd day after operation,the difference was not statistically significant(P>0.05).On the 5th and 7th day after operation,the swelling value of the affected leg in the two groups were lower than those on the 1st day after operation,and those in the treatment group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the VAS scores of the two groups were lower than those of before treatment,AOFAS scores were higher than those of before treatment,and the VAS scores of the treatment group was lower than that of the control group,AOFAS scores was higher than that of the control group,the differences were statistically significant(P<0.05).The incidence of complications in the treatment group was 6.67%,lower than 36.67%in the control group(P<0.05).The total effective rate of the treatment group was 93.33%,higher than 66.67%of the control group,the difference was statistically significant(χ^2=6.667,P=0.010).Conclusion:Huoxue Lishui method is a clinically effective treatment for limb swelling after Pilon fracture surgery,it can effectively reduce the calf swelling value,VAS score and complication rate,and improve the ankle joint activity score and therapeutic effect.
