摘要
目的探讨新型无创的单侧腹带加压固定装置对建立兔脊柱侧凸模型的可行性和有效性.方法对20只白兔采用右侧腹带加压外固定方法,定期更换加压装置,每次加压使外固定装置作用下主弯Cobb角增加10°,诱导前及诱导后定期摄后前位及侧位全脊柱X线片和CT平扫及三维重建,通过影像学观察兔脊柱弯曲在生长过程中的演变过程.测量影像学相关指标,采用SPSS21.0统计学软件对实验结果进行统计学分析.结果实验结束时,17只白兔成功建立脊柱侧凸模型.测量X线片诱导前Cobb角平均为0.5°(0°~1.0°),2周时Cobb角平均为2.8°(1.0°~6.0°),4周时Cobb角平均为10.6°(6.0°~16.0°),8周时Cobb角平均为20.4°(15.0°~26.0°),4个月时Cobb角平均27.3°(24.0°~31.0°),脊柱侧凸畸形随时间呈进行性加重.CT平扫显示胸廓两侧不对称,顶椎区域椎体出现旋转.三维重建示脊柱各椎体完整,未发生融合迹象.结论单侧腹带加压固定装置建立兔脊柱侧凸模型可行、无创、有效,值得推广.
Objective To explore the feasibility and validity of a new noninvasive unilateral abdominal band compression fixator for the establishment of scoliosis model in rabbits. Methods Twenty rabbits were treated with external fixation with pressure on the right ventral band, and the pressure device was replaced regularly. Each pressure increased the Cobb angle of the main bend by 10 degrees under the action of external fixator. X-ray and CT plain scan and three-dimensional reconstruction of anterior and lateral spine were completed before and after induction. The evolution of rabbit spinal curvature in the growth process was observed by imaging. Relevant imaging indicators were measured and the experimental results were analyzed by SPSS 21. 0 statistical software. Results At the end of the experiment, a scoliosis model was successfully established in 17 rabbits. The average Cobb angle before X-ray induction was 0. 5°(0°-1. 0°), 2. 8°(1. 0°-6. 0°) at 2 weeks, 10. 6°(6. 0°-16. 0°) at 4 weeks, 20. 4°(15. 0°-26. 0°) at 8 weeks and 27. 3°(24. 0°-31. 0°) at 4 months. The scoliosis deformity aggravated progressively with time. CT plain scan showed asymmetry on both sides of the thorax and rotation of the vertebral body in the apical region. Three-dimensional reconstruction showed that all vertebral bodies of the spine were intact without any sign of fusion. Conclusions It is feasible, noninvasive and effective to establish a model of scoliosis in rabbits with unilateral abdominal band compression fixator. It is worth popularizing.
