摘要
目的探讨清热养阴祛瘀饮辅助乌司他丁治疗脓毒血症疗效及对炎症因子、细胞免疫功能水平的影响。方法将60例患者随机分为对照组与观察组各30例,分别在对症干预基础上给予乌司他丁和清热养阴祛瘀饮+乌司他丁方案治疗,比较两组临床疗效、治疗前后中医证候积分、APACHEⅡ评分、MarshaⅡ评分、炎性细胞因子、细胞免疫功能指标水平及死亡率。结果观察组临床疗效显著优于对照组(P<0.05)。两组治疗后各症状积分均明显下降(P<0.05)。观察组治疗后中医证候积分显著低于对照组治疗后(P<0.05)。两组治疗后APACHEⅡ评分和MarshaⅡ评分均显著低于治疗前(P<0.05),且观察组治疗后评分更低(P<0.05)。两组治疗后TNF-α、CRP、IL-6均显著低于治疗前(P<0.05),且观察组治疗后炎性细胞因子水平更低(P<0.05)。两组治疗后CD3+、CD4+、CD4+/CD8+水平均显著高于治疗前,CD8+显著低于治疗前(均P<0.05),且观察组治疗后细胞免疫功能指标水平改善更明显(P<0.05)。同时观察组住院期间死亡率显著低于对照组(P<0.05)。结论清热养阴祛瘀饮辅助乌司他丁治疗脓毒血症能够显著缓解临床症状,控制病情进展,降低机体炎症反应水平,改善细胞免疫功能,并有助于提高存活率。
Objective:To investigate the clinical effects of Qingre Yangyin Quyu Decoction assisted with Ulinastatin on sepsis and its influence on inflammatory factor and cellular immune function.Methods:60 patients were randomly divided into the control group and the observation group,30 cases in each.On the basis of symptomatic intervention,the control group was treated with Ulinastatin,while the observation group was treated with Qingre Yangyin Quyu Decoction + Ulinastatin.The clinical efficacy,TCM syndrome score,APACHEⅡ score,MarshaⅡ score,inflammatory cytokines,cellular immune function index level and mortality were compared between the two groups before and after treatment.Results:The clinical efficacy of the observation group was significantly better than that of the control group(P < 0.05).The scores of symptoms in the observation group were significantly lower than those in the control group(P < 0.05).APACHEⅡ score and MarshaⅡ score were significantly lower than those before treatment(P < 0.05).The scores of the observation group was lower after treatment(P < 0.05).TNF-α, CRP and IL-6 were significantly lower than those before treatment(P < 0.05).The level of inflammatory cytokines in the observation group was lower after treatment(P < 0.05).After treatment,the levels of CD3+,CD4+ and CD4+/ CD8+ in both groups were significantly higher than those before treatment,and the levels of CD8+ were significantly lower than those before treatment(all P < 0.05).The level of cellular immune function in the observation group was improved more obviously after treatment(P < 0.05).At the same time,the mortality rate in the observation group was significantly lower than that in the control group(P < 0.05).Conclusion:Qingre Yangyin Quyu Decoctionassisted with Ulinastatin on sepsis can efficiently alleviate clinical symptoms,control the disease progress,reduce the levels of inflammatory reaction,and improve cellular immune function,helpful to higher survival rate.
