摘要
目的评估内镜下黏膜剥离术与黏膜切除术对上消化道早癌的疗效。方法将2017年1月-2019年1月的96例上消化道早癌的患者纳入研究范围,并应用随机数字表的方式对其平均分组,分别应用内镜下黏膜剥离术(试验组)进行治疗干预及黏膜切除术(对照组)进行干预,对比治疗效果。结果经研究,试验组患者的消化道穿孔、消化道出血和消化道狭窄的并发症发生人数为1例,对照组为10例,试验组并发症发生概率2.08%显著低于对照组20.83%,且试验组的一次性切除人数47例与完全切除人数48例多于对照组的36例和40例,试验组一次性切除率97.92%和完全切除率100%均显著高于对照组,P<0.05,差异有统计学意义。结论内镜下黏膜剥离术能够通过内镜对患者的消化道情况进行直观观察,治疗的安全性高,且治疗的成功率也比较高,疗效优于黏膜切除术。
Objective To evaluate the efficacy of endoscopic mucosal dissection and mucosal resection for early cancer of upper digestive tract. Methods 96 patients with early upper gastrointestinal cancer from January 2017 to January 2019 were enrolled in the study. The patients were divided into two groups by random number table, with average cases in each group. The treatment intervention group was treated by endoscopic mucosal dissection(the experimental group) and the control group was treated by mucosal resection(the control group). The therapeutic effect was compared. Results After treatment, the number of the experimental group patients with digestive tract perforation, gastrointestinal bleeding, and the digestive tract stenosis complications was 1 case, the control group was10 cases. The probability of complications in the experimental group was2.08% significantly lower than that in the control group(20.83%), and the number of one-time resections in the experimental group was 47 cases and the number of complete resections was 48 cases, which were more than those in the control group(36 cases and 40 cases). The one-time resection rate of the experimental group was 97.92% and complete resection rate was 100%, which were significantly higher than that in the control group, P<0.05, the difference was statistically significant. Conclusion Endoscopic submucosal dissection can directly observe the digestive tract of patients through endoscope. It has high safety, high success rate and better efficacy than mucosal resection.
