摘要
目的:系统评价正清风痛宁注射液治疗强直性脊柱炎的疗效与安全性。方法:检索CNKI、VIP、WanFang Data、The Cochrane Library、PubMed、EMbase数据库,收集正清风痛宁注射液治疗强直性脊柱炎的随机对照试验(RCT),检索时间截止到2017年12月。采用RevMan5.3软件进行统计分析。结果:纳入4项研究,共297例患者,其中治疗组150例,对照组147例。Meta分析结果显示,治疗组总有效率[RR=1.28,95%CI=(1.12,1.47),P=0.000,3]、枕壁试验[WMD=1.01,95%CI=(0.55,1.48),P<0.000,1]优于对照组;两组血沉[WMD=2.25,95%CI=(-2.40,6.90),P=0.34]、扩胸度[WMD=0.20,95%CI=(-0.15,0.55),P=0.27]比较,差异均无统计学意义。安全性方面,研究均未报告严重的不良反应事件。结论:正清风痛宁注射液(联合其他药物)治疗强直性脊柱炎,在改善枕壁试验和整体疗效方面具有明显优势,且未见有严重不良反应的报道。但由于纳入研究方法学质量较低、结局指标差异性大等原因,其潜在的疗效还需高质量的随机双盲对照试验来验证。
Objective: To systematically evaluate the clinical efficacy and safety of efficacy and safety of Zhengqing Fengtongning injection in the treatment of ankylosing spondylitis. Methods: We electronically searched databases including PubMed, the Cochrane Library, CNKI, Sinomed, EMbase, VIP, and WanFang Data to collect randomized controlled trials(RCTs) about Zhengqing Fengtongning injection in the treatment of ankylosing spondylitis from inception to Dec. 2017. Meta-analysis was performed by RevMan 5.3 software. Results: Four RCTs involving 297 patients were included, 150 cases in treatment group and 147 cases in control group. The results of meta-analysis showed that the treatment group showed better total effective rate [RR=1.28, 95%CI=(1.12, 1.47), P=0.000,3] and Occiput-wall trail [WMD=1.01,95%CI=(0.55,1.48), P<0.000,1] than control group.There was no statistical difference on ESR [WMD=2.25,95%CI=(-2.40,6.90), P=0.34] and degree of chest enlargement [WMD=0.20,95%CI=(-0.15,0.55), P=0.27] between the two groups. Conclusion: Current evidence shows,combined treatment shows better effects than Zhengqing Fengtongninginjection alone without medicine. We don't find any serious adverse reaction reports due to limited quality and quantity of the included studies, and more high quality studies are needed to verify the above conclusion.
