摘要
目的探讨Ⅱ型糖尿病并发视网膜病变(PDR)患者外周血炎症因子及血管内皮生长因子(VEGF)指标的变化情况。方法选取2016年10月至2018年9月胶州市人民医院治疗的216例Ⅱ型糖尿病患者的临床资料,根据患者是否发生PDR将其分为PDR组(n=102)与非PDR组(n=114)。检测并比较两组患者外周血相关指标和生化指标。外周血相关指标包括:C-反应蛋白(CRP)、血管内皮生长因子(VEGF)、白细胞介素-10(IL-10)与白细胞介素-6(IL-6)。生化指标包括总胆固醇(TC)、三酰甘油(TG)与糖化血红蛋白(HbA1c),每个指标检测3次取平均值。结果与非PDR组比较,PDR组患者外周血IL-6与VEGF水平升高(t=5.777、19.057,P均<0.01)。与非PDR组患者比较,PDR组患者HbA1c水平升高(t=16.570,P<0.01)。Pearson相关性检验表明,IL-6、HbA1c与VEGF呈正相关(r=0.714、0.676,P均<0.01)。结论Ⅱ型糖尿病合并PDR患者外周血VEGF水平显著升高,可能与促炎介质及高血糖共同导致患者发生眼部病变。
Objective To investigate the changes of inflammatory factors and vascular endothelial growth factor(VEGF)in patients with typeⅡdiabetes complicated with proliferative diabetic retinopathy(PDR).Methods The clinical data of 216 patients with typeⅡdiabetes treated in Jiaozhou People’s Hospital from October 2016 to September 2018 were selected.The patients were divided into PDR group(n=102)and non-PDR group(n=114)according to whether the patient had PDR.The peripheral blood related indicators and biochemical indicators of the two groups were detected and compared.Peripheral blood related indicators included:C-reactive protein(CRP),vascular endothelial growth factor(VEGF),interleukin-10(IL-10)and interleukin-6(IL-6).Biochemical indicators including total cholesterol(TC),triglyceride(TG)and hemoglobin a1c(HbA1c)were determined 3 times,and value of each indicator was averaged.Results Compared with the non-PDR group,the peripheral blood IL-6 and VEGF levels were significantly higher in the PDR group(t=5.777,19.057,P all<0.01).Compared with patients in the non-PDR group,the HbA1c level was significantly increased in the PDR group(t=16.570,P<0.01).Pearson correlation test showed that IL-6 and HbA1c levels were positively correlated with VEGF levels(r=0.714,0.676,P all<0.01).Conclusions The VEGF of peripheral blood in patients with typeⅡdiabetes mellitus complicated with retinopathy is significantly increased,which may be associated with inflammatory mediators and huperycemia leading to eye lesions in patients.
