摘要
目的探讨奥氮平联合重复经颅磁刺激治疗中度双相情感障碍抑郁发作的临床效果及对患者炎症因子及血清总胆红素的影响。方法将86例中度双相情感障碍抑郁发作患者根据治疗方案分为两组,每组43例。两组均口服奥氮平治疗,实验组在此基础上联合重复经颅磁刺激治疗,观察4周。采用汉密顿抑郁量表和贝克-拉范森躁狂量表评定抑郁躁狂状况,连线测验评定认知功能,检测并比较两组患者的血清炎症因子、白蛋白及总胆红素水平。结果与治疗前比较,治疗4周末两组汉密顿抑郁量表和贝克-拉范森躁狂量表评分和白细胞介素-1、肿瘤坏死因子-α水平显著降低(P<0.01),连线测验-A和连线测验-B完成时间显著缩短(P<0.01),白细胞介素-10及血清白蛋白、总胆红素水平显著升高(P<0.01);同期两组间比较,治疗前差异无统计学意义(P>0.05),治疗4周末差异有统计学意义(P<0.05或0.01)。结论奥氮平联合重复经颅磁刺激治疗中度双相情感障碍抑郁发作患者具有增效作用,疗效显著,能有效降低炎症因子水平,提高患者的认知功能。
Objective To investigate the clinical effect of olanzapine combined with rTMS on inflammatory factors and serum total bilirubin in patients with moderate bipolar affective disorder.Methods 86 patients with moderate bipolar affective disorder were divided into two groups,43 cases in each group.The control group was only given olanzapine treatment,and the experimental group was combined with rTMS on the basis of the control group.Patients were observed for 4 weeks.HAMD scale and BRMS scale were used to evaluate the emotional state of the patients,TMT was used to evaluate the changes in cognitive function.The levels of inflammatory factors,serum albumin and total bilirubin were measured and compared between the two groups.Results Compared with before treatment,the HAMD and BRMS scale scores,IL-1 and TNF-α levels of the two groups were significantly decreased after 4 weeks of treatment(P<0.01).TMT-A and TMT-B completion time was significantly shortened(P<0.01).Interleukin-10 and serum albumin and total bilirubin levels were significantly increased(P<0.01).There was no significant difference between the two groups(P>0.05),and the difference was statistically significant at the end of the 4th week(P<0.05 or 0.01).Conclusion Olanzapine combined with rTMS has a synergistic effect on moderate bipolar affective disorder.It can effectively reduce the level of inflammatory factors and improve the cognitive function of patients.
