摘要
本研究对国内外有关多学科团队协作的延续性护理文献进行搜索及学习,制定多学科团队协作在重症肺炎患儿延续性护理中应用的内容及实施方法,如建立多学科团队培训和考核体系、对患者信息实行统一管理、拓展应用智能手机APP平台等,以期达到降低患儿复发率及再住院率,提高患儿照护质量和生活质量的目的。
This paper reviewed studies on multi-disciplinary team cooperation in the continuity of care for children with severe pneumonia at home and abroad.The standardized content and implementation method for multi-disciplinary teams were also discussed,including establishment of training and evaluation system,unified management of patient information,application of smartphone-based APP,in order to reduce the risk of pneumonia-induced recurrence and rehospitalization,and to improve the continuity of care quality and quality of life in children with severe pneumonia.
