摘要
目的探究沙库巴曲缬沙坦联合胺碘酮治疗合并阵发性房颤的老年性心力衰竭的疗效。方法选择170例老年性心力衰竭合并阵发性房颤患者,随机分为观察组和对照组,每组85例。2组入院后均予以常规抗心力衰竭治疗。对照组加用胺碘酮治疗,观察组在此基础上联用沙库巴曲缬沙坦治疗,2组连续治疗12个月,比较2组临床疗效、窦性心律维持率、左心房内径、心率、左心室射血分数(LVEF)及用药安全性。结果观察组临床总有效率为92. 9%,高于对照组的70. 6%(P <0. 05)。观察组治疗后第6、9、12个月窦性心律维持率分别为88. 2%、82. 4%、76. 5%,均分别高于对照组的71. 8%、64. 7%、56. 5%(P <0. 05)。观察组治疗后左心房内径小于对照组(P <0. 05);观察组治疗后心率、LVEF均分别高于对照组(P <0. 05)。2组均未发生严重药物不良反应。结论对老年心力衰竭伴阵发性房颤患者予以沙库巴曲缬沙坦联合胺碘酮治疗,能取得更为满意的临床疗效,有助于提高窦性心律维持率,并对改善心功能,对心脏重构有较好的改善作用,且治疗安全性较高。
objective To investigate the therapeutic effects of sacubitril valsartan combined with amiodarone on senile heart failure complicated by paroxysmal atrial fibrillation.Methods A total of 170 elderly patients with heart failure complicated by paroxysmal atrial fibrillation who were admitted and treated in our hospital in recent years were randomly divided into observation group(n=85)and control group(n=85).The patients in both groups were treated by conventional anti-heart failure therapy,on this basis,the patients in control group were treated amiodarone,and the patients in observation group were treated by sacubitril valsartan combined with amiodarone,with a treatment course of 12 months for both groups.The clinical curative effects,sinus rhythm maintenance rate,left atrial diameter,heart rate,left ventricular ejection fraction(LVEF)and medication safety were observed and compared between the two groups.Results The total effective rate in observation group was 92.9%,which was significantly higher than that(70.6%)in control group(P<0.05).The maintenance rates of sinus rhythm at 6m,9m,12m after treatment in observation group were 88.2%,82.4%and 76.5%,respectively,which were significantly higher than those(71.8%,64.7%and 56.5%,respectively)in control group(P<0.05).Moreover the left atrial diameter in observation group was significantly less than that in control group(P<0.05).The heart rate and LVEF in observation group were significantly higher than those in control group(P<0.05).No serious adverse drug reactions were observed in both groups.Conclusion The sacubitril valsartan combined with amiodarone in treatment of senile heart failure complicated by paroxysmal atrial fibrillation can obtain satisfactory therapeutic effects,which is helpful to improve the maintenance rate of sinus rhythm,improve cardiac function,improve cardiac remodeling,with good medication safety.
