摘要
背景:目前研究发现脂质代谢异常、脂肪肝与结直肠息肉的发生具有相关性,但仍有争议。肝脏受控衰减参数(CAP)在评估肝脏脂肪变程度中具有良好的应用价值。目的:分析脂质代谢和肝脏CAP与结直肠息肉的相关性。方法:选取2016年1月—2018年1月于青岛市市立医院行电子结肠镜检查的患者,根据检查结果分为结直肠息肉组和对照组。比较两组总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)、脂蛋白a、CAP、BMI,并分析脂质代谢指标与CAP的相关性。结果:与对照组相比,结直肠息肉组TG异常的发生率、CAP、BMI均显著升高(P <0. 05);而两组血清TC、HDL、LDL、Apo A1、Apo B、脂蛋白a水平异常发生率无明显差异(P> 0. 05)。结直肠息肉患者CAP与TG、LDL、BMI呈正相关(r=0. 201,r=0. 135,r=0. 438; P均<0. 05),而与TC、HDL无关(P> 0. 05)。结论:TG、CAP、BMI可能与结直肠息肉的发生相关。
Background: Studies have shown that lipid metabolism disorders and fatty liver disease are related to the pathogenesis of colorectal polyps,but it is still in controversial.Controlled attenuation parameter (CAP) of liver has value in assessing the degree of hepatic steatosis.Aims:To investigate the correlation of lipid metabolism and CAPof liver with colorectal polyps.Methods:Patients undergone colonoscopy from January 2016 to January 2018 at Qingdao Municipal Hospital were enrolled.And these patients were divided into colorectal polyps grouPand control grouPaccording to the results of colonoscopy.The levels of total cholesterol (TC),triglyceride (TG),high density lipoprotein (HDL),low density lipoprotein (LDL),apolipoprotein A1 (ApoA1),apolipoprotein B (ApoB),lipoprotein a,CAP,BMI were compared between the two groups.The correlation of CAPwith lipid metabolism parameters in patients with colorectal polyps was analyzed.Results:Compared with control group,the incidence of TG abnormality,and levels of CAP,BMI were significantly higher in colorectal polyps patients ( P<0.05).However,no significant differences in the incidences of abnormality of TC,HDL,LDL,ApoA1,ApoB,lipoprotein a were found between the two groups ( P>0.05).Levels of TG,LDL,BMI were positively correlated with CAPin patients with colorectal polyps ( r=0.201,r=0.135,r=0.438; Pall <0.05).However,TC,HDL was not correlated with CAP( P>0.05).Conclusions:TG,CAP,BMI may be correlated with the incidence of colorectal polyps.
