前列腺癌患者全身骨显像骨转移因素分析 被引量:4

Analysis of osseous metastasis factors of patients with prostate cancer by whole body bone imaging

摘要 目的:探讨前列腺癌诊断因素与全身骨显像骨转移之间的关系。方法:回顾性分析227例确诊为前列腺癌患者的全身骨显像资料,以全身骨显像结果为诊断骨转移的金标准,采用单因素Pearson卡方分析和logistic二分类回归方法对前列腺癌骨转移的相关因素总前列腺癌特异抗原(TPSA)、游离前列腺特异性抗原(FPSA)、游离前列腺癌特异抗原与总前列腺癌特异抗原比[RAT(F/T)]、前列腺癌Gleason评分(GS)、碱性磷酸酶(ALP)及盆腔淋巴结进行分析。结果:经Pearson卡方统计分析,RAT(F/T)与骨转移无相关性(x^2=0.873,P>0.05);而TPSA、FPSA、GS、ALP及盆腔淋巴节均与骨转移相关(x^2=91.186,x^2=32.063,x^2=39.807,x^2=73.356,x^2=58.925;P<0.05)。经logistic二分类回归分析,TPSA、ALP、GS和盆腔淋巴均与骨转移有相关性(OR=1.019,OR=1.030,OR=1.521,OR=10.241;P<0.05);RAT(F/T)和FPSA与骨转移无统计学意义(OR=1.034,OR=0.982;P>0.05),均不是骨转移的危险因素。结论:TPSA、GS、ALP及盆腔淋巴结是前列腺癌骨转移的重要因素;FPSA与骨转移有一定关系,但不是骨转移的危险因素;RAT(F/T)与骨转移无关。 Objective:To investigate the relationship between the diagnostic factors of prostate cancer and osseous metastasis by whole body bone imaging.Method:The data of whole body bone imaging of 227 patients with prostate cancer were analyzed retrospectively.The results of whole body bone imaging were used as the gold standard for the diagnosis of bone metastasis.One-factor Pearson Chi-square analysis and Binary Logistic regression were used to analyze correlative factors of bone metastasis of prostate cancer included total prostate cancer specific antigen(TPSA),free prostate cancer specific antigen(FPSA),the ratio of free prostate cancer specific antigen(FPSA)and total prostate cancer specific antigen RAT(F/T),Gleason score of prostate cancer,alkaline phosphatase(ALP)and pelvic lymph nodes.Results:The result of one-factor Pearson Chi-square analysis indicated that there was no correlation between bone metastasis and RAT(F/T)(x^2=0.873,P>0.05),while TPSA,FPSA,GS,ALP and the pelvic lymph node were correlative with bone metastasis(x^2=91.186,x^2=32.063,x^2=39.807,x^2=73.356,x^2=58.925,P<0.05),respectively.The results of Binary Logistic regression analysis indicated that T-PSA,ALP,GS and the pelvic lymph node were correlative with bone metastasis(OR=1.019,OR=1.030,OR=1.521,OR=10.241,P<0.05),while the correlations of RAT(F/T)and FPSA with bone metastasis were not statistically significant(OR=1.034,OR=0.982,P>0.05),respectively,and they were not risk factors of bone metastasis.Conclusion:TPSA,GS,ALP and the pelvic lymph node are important factors of bone metastasis of prostate cancer.FPSA has a certainly relationship with bone metastasis,but it is not risk factor.And RAT(F/T)is not correlative with bone metastasis.
