摘要
目的分析固定义齿修复后并发牙龈炎的影响因素、牙周指数、细菌分布以及治疗情况。方法选取本院2016年11月~2017年11月收治的56例(63颗)固定义齿修复患者作为研究对象,对其临床资料进行回顾性分析,观察患者修复后并发牙龈炎的情况。结果固定义齿修复后出现牙龈炎51颗,发病率为81.0%,其影响因素主要为冠颈部与肩台的密合性因素,占比51.0%,其后依次是桥体因素、粘结剂因素、修复体积邻牙因素以及冠外形因素。牙龈炎患者的菌斑指数为(1.8±0.7),牙周治疗需要指数为(2.0±0.5),探诊深度为(3.5±1.1)mm;细菌分布中,球菌(38.2±2.1),螺旋体(10.5±3.0),直杆菌(22.4±3.5),丝状菌(11.2±1.9),梭状菌(10.5±1.8),弯曲菌(9.9±1.6)。51颗牙龈炎经治疗,总有效率为72.5%,粘结剂因素所致牙龈炎的总有效率最高,为100.0%,其后依次是冠颈部与肩台的密合性因素、修复体积邻牙因素、桥体因素与冠外形因素。结论固定义齿修复后并发牙龈炎的可能性较大,应全面分析其影响因素,积极进行预防与治疗。
Objective To analyze the influential factors,periodontal index,bacterial distribution and treatment of gingivitis after fixed denture restoration.Methods 56 patients(63)with fixed dentures from November 2016 to November 2017 were selected as the research subjects.The clinical data of the patients were analyzed retrospectively,and the cases of gingivitis were observed after the patients were repaired.Results 51 gingivitis were found after the fixed denture,the incidence was 81.0%.The main factors were the close combination of the crown and neck and shoulder,which accounted for 51.0%,followed by the bridge body factor,the adhesive factor,the repair volume of the adjacent teeth factors and the crown shape factors.The plaque index of the patients with gingivitis was(1.8±0.7),the need index of periodontal treatment was(2.0±0.5),the depth of probing was(3.5±1.1)mm,the bacteria distribution was cocci(38.2±2.1),spirochetes(10.5±3.0),straight bacilli(22.4±3.5),filamentous bacteria(11.2±1.9),Clostridium(10.5±1.8),campylobacter(9.9±1.6).The total effective rate of 51 gingivitis was 72.5%.The total effective rate of gingivitis caused by adhesive factors was the highest,which was 100.0%,followed by the close relationship between the crown and neck and the shoulder,the repair of the volume of the adjacent teeth,the factors of the bridge body and the factor of the crown shape.Conclusion The possibility of gingivitis after fixed partial denture is relatively large.We should comprehensively analyze its influencing factors and actively carry out prevention and treatment.
