血气分析联合血清BNP对慢性阻塞性肺疾病急性加重期合并心力衰竭诊断价值的分析 被引量:4

Analysis of Value of Blood Gas Analysis Combined with Serum BNP in Diagnosis of AECO-PD and Heart Failure

摘要 目的探讨血气分析联合血清B-型脑钠肽(BNP)对慢性阻塞性肺疾病(COPD)急性加重期合并心力衰竭的临床诊断。方法选取2014年1月—2017年1月该院收治的COPD急性加重期患者85例,按是否合并心力衰竭分为研究组(合并心力衰竭,n=46例)和对照组(未合并心力衰竭,n=39例),均进行血气分析、血清BNP检测,其中血气分析指标包括动脉血压分压(Pa O2)、动脉血二氧化碳分压(Pa CO2),并分析血清BNP与动脉血气指标的相关性。结果研究组Pa O2水平为(41.05±3.67)mm Hg,显著低于对照组(64.39±4.12)mm Hg,研究组Pa CO2水平为(84.38±6.89)mm Hg,显著高于对照组(52.87±5.76)mm Hg,差异有统计学意义(P<0.05);研究组血清BNP水平为(1 065.28±175.34)p/L,显著高于对照组(512.87±113.17)p/L,差异有统计学意义(P<0.05);经Pearson相关性分析,血清BNP与Pa O2呈负相关(r=-0.425,P<0.05),与Pa CO2呈正相关(r=0.651,P<0.05)。结论血清BNP能反映COPD患者病情严重程度,采取血气分析联合血清BNP检测的方式,对COPD急性加重期是否合并心力衰竭的判断有一定的临床价值。 Objective To study the value of blood gas analysis combined with serum BNP in diagnosis of AECO-PD and heart failure.Methods 85 cases of patients with AECO-PD admitted and treated in our hospital from January 2014 to January 2017 were selected and divided into the research group with 46 cases(with heart failure)and control group with 39 cases(without heart failure),and both groups were for blood gas analysis and serum BNP,and the blood gas analysis indicators included the PaO2 and PaCO2,and the correlation between the serum BNP and arterial blood gas indicators was analyzed.Results The PaO2 level in the research group was obviously lower than that in the control group,[(41.05±3.67)mmHg vs(64.39±4.12)mmHg],and the PaCO2 level in the research group was obviously higher than that in the control group[(84.38±6.89)mmHg vs(52.87±5.76)mmHg],and the difference was statistically significant(P<0.05),and the serum BNP level in the research group was obviously higher than that in the control group,[(1 065.28±175.34)p/L vs(512.87±113.17)p/L],and the difference was statistically significant,P<0.05,and the Pearson correlated analysis showed that the serum BNP was negatively correlated with PaO2(r=-0.425,P<0.05),positively correlated with PaCO2(r=0.651,P<0.05).Conclusion The serum BNP can reflect the severity of COPD patients,and the blood gas analysis combined with serum BNP test has a certain clinical value to the determination of AECO-PD and heart failure.
