摘要
目的观察慢性间质性肾炎(chronic interstitial nephritis,CIN)患者在糖皮质激素治疗前后尿液生物学指标的变化。方法临床诊断为CIN的患者29例,采用甲基强的松为主的治疗,疗程3个月。观察治疗效果、治疗前后患者肾功能指标的变化,以及尿液转化生长因子β1(TGF-β1)、巨噬细胞炎性蛋白-1α(MIP-1α)、中性粒细胞明胶酶相关性脂质运载蛋白(NGAL)水平的变化。结果经过3个月治疗后患者肾功能指标整体改善,其中疾病好转11例,疾病稳定13例,恶化5例。患者尿液TGF-β1、MIP-1α和NGAL与治疗前比较显著降低(P<0.05),其中疾病恶化患者的尿液TGF-β1、MIP-1α和NGAL水平显著高于疾病好转和稳定患者。结论尿液生物学指标TGF-β1、MIP-1α和NGAL可以反映CIN的激素治疗效果,且其水平变化可以反映疾病进展情况,有助于评估患者疗效。
Objective To observe the changes of urine biomarkers in patients with chronic interstitial nephritis(CIN)before and after glucocorticoid treatment. Methods Twenty-nine patients diagnosed clinically with CIN were treated mainly with methyl-prednisone for 3 months.The clinical effects,the renal function indexes changes before and after treatment,as well as the changes of transforming growth factorβ1(TGF-β1),macrophage inflammatory protein-1α(MIP-1α)and neutrophil gelatinase-associated lipocalin(NGAL)were observed. Results After 3 months of treatment,the renal functions of the patients were improved overall,among them 11 cases were improved,13 cases were stable,and 5 cases were worsened.After the treatment,urine TGF-β1,MIP-1αand NGAL were significantly lower than those before treatment(P<0.05).Moreover,the levels of urine TGF-β1,MIP-1αand NGAL in patients whose diseases became worsened were significantly higher than those in patients whose diseases were improved and stable. Conclusion Urinary biomarkers TGF-β1,MIP-1αand NGAL can reflect the effects of hormone in treatment of CIN,and its level change reflects the progression of the disease,which helps to assess the treatment efficacy.
