摘要
目的探讨结核感染T细胞斑点(T-SPOT.TB)试验在脊柱结核诊断中的应用价值。方法将2015年1月至2016年12月佛山市南海区人民医院收治并确诊的脊柱结核与非结核各50例患者,分为结核组与非结核组。两组患者均行T-SPOT.TB检测、结核菌素纯蛋白衍化物(PPD)皮试、抗结核抗体检测,比较3种诊断方法的检测结果。结果 T-SPOT.TB试验检测脊柱结核的敏感度为88%,特异度为94%,阳性似然比为14.667,阴性似然比为0.126,明显优于PPD皮试和抗结核抗体检测;对脊柱结核组T-SPOT.TB检测与其他两种检测方法进行配对比较,差异亦有统计学意义(P<0.05)。结论 T-SPOT.TB试验诊断脊柱结核灵敏度及特异度较高,检测结果真实可靠,具有较好的应用价值。
Objective To explore the applicable value of T-cell spot of tuberculosis(T-SPOT.TB)assay in the diagnosis of spinal tuberculosis.Methods Fifty patients with spinal tuberculosis and 50 non-tuberculosis patients treated from January 2015 to December 2016 in Nanhai District People's Hospital in Foshan City were selected and then divided into tuberculosis and non-tuberculosis groups respectively.All patients underwent T-SPOT.TB detection,tuberculin purified protein derivative(PPD)skin test and tuberculosis antibody(TB-Ab)test,and the examination results were compared among 3 diagnostic methods.Results The sensitivity,specificity,positive likelihood ratio and negative likelihood ratio of T-SPOT.TB detection was 88%,94%,14.667 and 0.126 respectively,which were superior than those in PPD skin test and TB-Ab test;For tuberculosis group,comparison of matched pairs among T-SPOT.TB assay and other two detection methods showed the differences were statistically significant(P<0.05).Conclusion T-SPOT.TB assay has high sensitivity and specificity in the diagnosis of spinal tuberculosis,and the results of the detection are reliable,which is worthy to being recommended in clinical application.
