摘要
目的探讨布地奈德气雾剂雾化吸入联合无创正压通气(NIPPV)对慢性阻塞性肺炎(COPD)合并Ⅱ型呼吸衰竭患者动脉血气指标改善及肺功能的影响。方法选取漯河市郾城区人民医院2014年5月至2016年5月收治的COPD合并Ⅱ型呼吸衰竭患者62例,按照随机数字表法分为对照组和观察组,各31例,对照组采用NIPPV治疗,观察组于此基础上联合布地奈德雾剂雾化吸入治疗,均治疗35 d。对比两组治疗效果,检测并统计两组治疗前后血气指标、肺功能指标及治疗前后两组生活质量评分。结果观察组总有效率93.55%高于对照组67.74%,差异有统计学意义(P<0.05);治疗后,观察组动脉氧分压(PaO_2)及血氧饱和度(SaO_2)高于对照组,动脉二氧化碳分压(PaCO_2)低于对照组,差异有统计学意义(P<0.05);治疗后观察组用力肺活量(FVC)、第1秒用力呼吸容积(FEV1)及FEV1/FVC高于对照组,观察组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论布地奈德气雾剂雾化吸入联合NIPPV治疗COPD合并Ⅱ型呼吸衰竭,可有效改善患者动脉血气指标及肺功能,同时有助于提升其生活质量,疗效显著。
Objective To investigate Budesonide Aerosol inhalation combined noninvasive positive pressure ventilation(NIP原PVon chronic obstructive pulmonary disease(COPDof patients with type II respiratory failure to improve arterial blood gas and pul原monary function.Methods We selected62patients COPD with respiratory failure in LuoheYancheng District People's Hospital from May2014to May2016.According to the random number table method,they were divided into the control group and the obser原vation group,each31cases.The control group was treated with NIPPV,and the observation group was treated with Budesonide aerosol inhalation on the basis of this treatment,all were treated35days.The therapeutic effects of the two groups were compared and the blood gas indexes,pulmonary function indexes and the quality of life scores of the two groups before and after treatment were detected and counted before and after treatment in the two groups.Results The total effective rate of the observation group was93.55%(29/31),higher than that of the control group67.74%(21/31),and the difference was statistically significant(P约0.05);After treatment,the arterial oxygen partial pressure(PaO2and blood oxygen saturation(SaO2of the observation group were higher than that of the control group,and the arterial carbon dioxide pressure(PaCO2of the observation group was lower than that of the control group,and the difference was statistically significant(P约0.05);After treatment,the forced vital capacity(FVC),forced expira原tory volume(FEV1and FEV1/FVC in the first second of the observation group were higher than those in the control group.The quali原ty of life score in observation group was higher than that in control group,and the difference was statistically significant(P约0.05);ty of life score in observation group was higher than that in control group,and the difference was statistically significant(P约0.05);Conclusion Budesonide Aerosol aerosol inhalation combined with NIPPV can effectively improve the arterial blood gas index and lung function in patients with COPD combined with type II respiratory failure,and it can also improve the quality of life and the cu原rative effect is remarkable.
