多索茶碱联合噻托溴铵对COPD患者血清APN、NE及α1-AT水平的影响 被引量:6

Effect of doxofylline combined with tiotropium bromide on serum APN NE andα1-AT levels in COPD patients

摘要 目的探讨多索茶碱联合噻托溴铵对慢性阻塞性肺疾病(COPD)患者血清脂联素(APN)、中性粒细胞弹性蛋白酶(NE)及α1-抗胰蛋白酶(α1-AT)水平的影响。方法选取2020年2月—2022年5月重庆医科大学附属大足医院收治的110例COPD患者为研究对象,采用随机数字表法分为观察组与对照组,每组55例。对照组患者采用噻托溴铵治疗,观察组患者采用多索茶碱联合噻托溴铵进行治疗,比较2组患者的临床疗效,治疗前后的血清指标、肺功能指标、炎症指标水平以及不良反应发生情况。结果观察组患者的治疗总有效率为96.36%(53/55),高于对照组的80.00%(44/55),差异有统计学意义(χ^(2)=7.066,P=0.008)。治疗前,2组患者第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占预计值百分比(FEV_(1)%pred)、第1秒用力呼气容积/用力肺活量(FEV_(1)/FVC)水平比较,差异均无统计学意义(P>0.05);治疗后,2组患者FEV_(1)、FEV_(1)%pred、FEV_(1)/FVC水平均高于治疗前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。治疗前,2组患者血清APN、NE、α1-AT水平比较,差异均无统计学意义(P>0.05);治疗后,2组患者血清APN、α1-AT水平均高于治疗前且观察组均高于对照组,血清NE水平均低于治疗前且观察组低于对照组,差异均有统计学意义(P<0.05)。治疗前,2组患者血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平比较,差异均无统计学意义(P>0.05);治疗后,2组患者血清IL-6、IL-8、TNF-α水平均低于治疗前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。观察组与对照组患者不良反应发生率分别为14.55%(8/55)和18.18%(10/55),差异无统计学意义(χ^(2)=0.266,P=0.606)。结论多索茶碱联合噻托溴铵治疗COPD,有助于提高患者肺通气功能,改善血清指标水平,减轻炎症反应,且安全性较高。 Objective This paper aims to investigate the effect of doxofylline combined with tiotropium bromide on serum adiponectin(APN),neutrophil elastase(NE),andα1-antitrypsin(α1-AT)levels in patients with chronic obstructive pulmonary disease(COPD).Methods Overall,110 patients with COPD admitted to Dazu Hospital of Chongqing Medical University from Feb.2020 to May 2022 were selected as the study participants,who were assigned to the observation group(n=55)and the control group(n=55)by a random number table.The patients in the control group received tiotropium bromide;while the patients in the observation group,doxofylline combined with tiotropium bromide.The clinical efficacy,serum indicators,pulmonary function indicators,inflammatory indicators,and adverse reactions before and after treatment were compared between the two groups.Results The overall response rate of the observation group was significantly higher than that of the control group[96.36%(53/55)vs.80.00%(44/55);χ^(2)=7.066,P=0.008].Before treatment,no significant difference was observed in forced expiratory volume in 1 second(FEV_(1)),predicted percentage of forced expiratory volume in 1 second(FEV_(1)%pred),and ratio of forced expiratory volume in 1 second to forced vital capacity(FEV_(1)/FVC)levels between the two groups(P>0.05);FEV_(1),FEV_(1)%pred and FEV_(1)/FVC levels in the two groups showed a significant increase after treatment,and the observation group was significantly higher than the control group(P<0.05).Before treatment,no significant difference was found in serum APN,NE,andα1-AT levels between the two groups(P>0.05);serum APN andα1-AT levels in the two groups were significantly increased after treatment,and the observation group was significantly higher than the control group(P<0.05);serum NE levels were significantly declined,and the observation group was significantly lower than that in the control group(P<0.05).Before treatment,no significant difference was detected in serum interleukin-6(IL-6),interleukin-8(IL-8),and tumor necrosis factor-α(TNF-α)levels between the two groups(P>0.05);serum IL-6,IL-8 and TNF-αlevels in the two groups were decreased after treatment,and the observation group was lower than the control group,with statistical significance(P<0.05).The incidence of adverse reactions in the observation group and the control group was not statistically significant[14.55%(8/55)vs.18.18%(10/55),χ^(2)=0.266,P=0.606].Conclusion Doxofylline in combination with tiotropium bromide in the treatment of COPD patients is beneficial to promoting pulmonary function,potentiating serum indicators levels,and reducing the inflammatory response,with high safety profiles.
