R95活性炭呼吸器对减少氧化产物吸入的作用

摘要 目的:探讨佩戴R95活性炭呼吸器是否可以减少多环芳香烃(PAH)的摄入量,分析R95活性炭呼吸器对减少氧化产物吸入的作用,方法:选取44名街道工作人员,将其按照数表发随机分为观察组和对照组组,每组22名。观察组连续2 d工作时间佩戴呼吸器,对照组则连续2 d工作时间未佩戴呼吸器,运用气相色谱法和(或)高分辨质谱法检测羟基多环芳烃(OH-PAH)。结果:在两组受试者中,出租车司机占38.6%,街头小贩占34.1%。尿液中1-羟基芘浓度(1-OHP)的基线肌酐校正水平比其他国际标准高:1020 ng/g肌酐(IQR:604~1551)。佩戴R95活性炭呼吸器对1-OHP水平无显著影响,乘数效应1.00(95%CI:0.92~1.09);对其他OH-PAHs亦无显著影响,1-羟基萘(1-OHN)例外,乘数效应0.86(95%CI:0.77~0.96)。结论:街道工作者的尿PAH含量较高。在大城市中空气高度污染的地区内,佩戴R95活性炭呼吸器并不会影响尿OH-PAH水平,除1-OHN例外。缺乏效果可能是因为R95活性炭呼吸器无法有效地过滤气相PAHs。此外,检测出的高浓度尿OH-PAHs,提示采取有效干预措施的必要性。 Objective:To investigate whether respirator with R95activated carbon could reduce the intake of polycyclic aromatic hydrocarbon(PAH)so as to analyze the effect of respirator with R95activated carbon in reducing the breath-in of oxidative product.Methods:44subjects were divided into wearing respirator group(22cases)and un-wearing respirator group(22cases).The experiment period was2d,and the gas chromatographic method and(or)high resolution mass spectrum were adopted to detect hydroxy PAH(OH-PAH).Results:In44subjects,the ratios of taxi driver was38.6%and street hawker was34.1%,respectively.In urine,the corrected level of baseline creatinine of1-OHP concentration was higher than that of international standards:1020ng/g creatinine(IQR:604-1551).Whether wearing respirator with R95activated carbon didn't has significant effect on1-OHP level,and its multiplier effect was1.00(95%CI:0.92-1.09).Besides,it also didn't has significant effect on other OH-PAHs except1-OHN,and its multiplier effect was0.86(95%CI:0.77-0.96).Conclusion:In street workers,the OH-PAHs level in urine was higher.In the area with severe air pollution,wearing respirator with R95activated carbon don't influence the OH-PAH level in urine except1-OHN.The reasons might be respirator with R95activated carbon couldn't efficiently filter PAHs.Besides,the OH-PAHs of high concentration indicates that it is necessary to adopt effective intervention.
