沙丁胺醇联合布地奈德氧气雾化吸入治疗支气管哮喘急性发作的疗效观察 被引量:8

Observation of curative effect by salbutamol combined with budesonide by oxygen aerosol inhalation in the treatment of acute episode of bronchial asthma

摘要 目的探讨沙丁胺醇布地奈德氧气雾化吸入治疗支气管哮喘急性发作的疗效及安全性。方法 80例支气管哮喘急性发作患者,根据随机数字表法分为联合组和沙丁胺醇组,各40例。沙丁胺醇组在常规治疗基础上给予硫酸沙丁胺醇片和地塞米松;联合组在常规治疗基础上给予沙丁胺醇雾化液、布地奈德雾化液经氧气雾化吸入治疗。比较两组患者临床症状、体征缓解时间,治疗前后肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC和峰值呼气流速占预计值的百分比(PEF%)],临床疗效及不良反应发生情况。结果联合组气促、胸闷、咳嗽、哮鸣音缓解时间均明显短于沙丁胺醇组,差异具有统计学意义(P<0.05);治疗后,联合组患者FEV1、FEV1/FVC、PEF%明显高于沙丁胺醇组,差异具有统计学意义(P<0.05);联合组临床控制率为55.0%、总有效率为90.0%,均明显高于沙丁胺醇组的37.5%、72.5%,差异有统计学意义(P<0.05)。两组治疗期间均未出现明显不良反应。结论布地奈德、沙丁胺醇雾化吸入治疗支气管哮喘急性发作,能及时缓解哮喘相关症状、改善肺功能,提高临床正常控制率,且安全可靠,值得临床推广应用。 Objective To investigate curative effect and safety by salbutamol combined with budesonide by oxygen aerosol inhalation in the treatment of acute episode of bronchial asthma. Methods A total of 80 patients with acute episode of bronchial asthma were divided by random number table into combined group and salbutamol group, with 40 cases in each group. The salbutamol group received salbutamol and dexamethasone on the basis of conventional therapy, and the combined group received additional salbutamol atomized liquid and budesonide atomized liquid by oxygen aerosol inhalation to conventional therapy. Comparison was made on clinical symptoms, vital signs remission time, pulmonary function indexes before and after treatment[ forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), FEV1/FVC and percent of expected peak expiratory flow (PEF%)], clinical effects and adverse reactions between the two groups. Results The combined group had obviously shorter remission time of anhelation, chest distress, cough and wheezing than the salbutamol group, and their difference had statistical significance (P<0.05). After treatment, the combined group had much higher FEV1,FEV1/FVC and PEF% than the salbutamol group, and the difference had statistical significance (P<0.05). The combined group had clinical control rate as 55.0% and total effective rate as 90.0%, which were much higher than 37.5% and 72.5% in the salbutamol group, and their difference had statistical significance (P<0.05). There was no obvious adverse reaction in both groups during treatment. Conclusion Oxygen aerosol inhalation of budesonide and salbutamol in treating acute episode of bronchial asthma can timely relieve asthma-related symptoms, improve pulmonary function, and increase normal clinical control rate, along with safety and creditability. This method is worth clinical promotion and application.
