摘要
[目的]观察益肾健脾方联合西药治疗慢性肾小球肾炎(脾肾阳虚)疗效。[方法]使用随机平行对照方法,将60例住院及门诊患者按随机数字表方法随机分为两组,合并高血压,左旋氨氯地平,2.5mg/次,1次/d;水肿,呋塞米,20mg/次,2次/d,或配螺内酯,20mg/次,2次/d;并发感染,抗生素。对照组30例肾炎康复片,6片/次,3次/d。治疗组30例益肾健脾方(黄芪30g,~怀牛膝、~怀山药各20g,地龙10g,杜仲、枸杞、淫羊藿、泽兰各20g,白术10g,山茱萸、丹参各20g,红花、大黄~生各5g,玉米须20g),水煎200mL,1剂/d,早晚温服。连续治疗4周为1疗程。观测临床表现、尿蛋白、血肌酐、血尿素氮、血红蛋白、不良反应。连续治疗2疗程(8周),判定疗效。[结果]治疗组临床控制8例,显效11例,有效9例,无效2例,总有效率93.33%;对照组临床控制3例,显效9例,有效11例,无效7例,总有效率76.67%;两组无显著差异(P>0.05)。尿蛋白、血肌酐、血尿素氮、血红蛋白两组均有改善(P<0.01),尿蛋白、血肌酐、血尿素氮治疗组改善优于对照组(P<0.01),血红蛋白两组无显著差异(P>0.05)。[结论]益肾健脾方联合西药治疗慢性肾小球肾炎(脾肾阳虚),疗效满意,无严重不良反应,可明显改善肾功能,值得推广。
[Objective] To observe the therapeutic effect of Yishen Jianpi Decoction combined with western medicine on chronic glomerulonephritis（spleen and kidney Yang deficiency）. [Methods] 60 inpatients and outpatients were randomly divided into two groups according to the random number table, including hypertension, levamlodipine, 2.5 mg/time, once per day, edema, furosemide, 20 mg/time, twice a day,or spironolactone, 20 mg/time, twice a day, infection and antibiotics. Control group 30 cases of nephritis rehabilitation tablets, 6 tablets/times, 3 times/d. In the treatment group, 30 cases were treated with Yishen Jianpi decoction（Huangqi 30 g, Huainiuxi, Huaishanyao 20 g, DiLong 10 g, Duzhong,Gouqi, Yinyanghuo,Zelan each 20 g, Baizhu 10 g, Shanzhuyu, Danshen each 10 g, Honghua, Shengdahuang each 5 g, Yumixu 20 g）,decocted with water for 200 mL, 1 dose per day, taken in the morning and evening continuous treatment for 4 weeks was 1 courses. Clinical manifestations, urine protein, serum creatinine, blood urea nitrogen, hemoglobin and adverse reactions were observed. Continuous treatment for 2 courses（8 weeks）, to determine the curative effect. [Results] The total effective rate was 93.33% in the treatment group and 76.67% in the control group.There was no significant difference between the two groups（P〉0.05）. Urine protein, serum creatinine, blood urea nitrogen and hemoglobin were improved in both groups（P〈0.01）. The improvement of urine protein,serum creatinine and blood urea nitrogen in the treatment group was better than that in the control group（P〈0.01）. There was no significant difference in hemoglobin between the two groups（P〉0.05）. [Conclusion]Yishen Jianpi decoction combined with western medicine in the treatment of chronic glomerulonephritis（Pishen Yangxu） has satisfactory curative effect and no serious adverse reactions. It can obviously improve renal function and is worth popularizing.
