摘要
目的探讨缺铁性贫血患者糖化血红蛋白(HbA1c)检测结果的变化,以及缺铁性贫血对HbA1c检测值的影响,为临床诊治糖尿病提供更精准的指导依据。方法对87例缺铁性贫血患者进行血细胞分析及铁蛋白、空腹血糖(FBG)、HbA1c等检测,同时与100例健康对照者检测结果进行比较。结果缺铁性贫血患者与对照组相比,FBG差异无统计学意义(P>0.05);血红蛋白(Hb)、红细胞计数(RBC)、红细胞压积(HCT)、平均红细胞体积(MCV)、平均红细胞血红蛋白量(MCH)、平均红细胞血红蛋白浓度(MCHC)、血清铁蛋白等显著降低(P<0.05);HbA1c显著增高(P<0.05);健康对照组HbA1c与FBG呈正相关(r=0.916,P<0.05);缺铁性贫血患者HbA1c与FBG无相关性(r=0.429,P>0.05)。结论缺铁性贫血患者HbA1c水平升高,且与血糖水平无关,不能准确代表体内血糖水平,临床应予重视。
Objective To discuss the change of glycosylated hemoglobin (HbA1c) in patients with iron deficiency anemia (IDA),and effect of IDA on the HbA1c.And the research was to provide more accurate guidance basis for the clinical diagnosis and treatment of diabetes. Methods Iron protein,fasting blood glucose and glycosylated hemoglobin were detected in 87 IDA patients.And 100 healthy controls were selected as control group. Results There was no significant difference between IDA patients and control group in fasting blood glucose( P 〉0.05).Hemoglobin (Hb),red blood cell count (RBC),hematocrit (HCT),average red blood cell volume (MCV),average red blood cell hemoglobin content (MCH),average RBC hemoglobin concentration (MCHC),ferritin (Fer) of IDA patients were significantly lower than that of control group ( P 〈0.05).HbA1c was significantly higher than that of control group ( P 〈0.05).HbA1c was positively correlated with fasting blood glucose in healthy control group ( r =0.916, P 〈0.05).While,HbA1c was not correlated with fasting blood glucose in IDA patients ( r =0.429, P 〉0.05). Conclusion The results of the HbA1c in IDA patients are elevated,and it is not related to blood glucose level.It may not accurately represent the blood glucose level in the body,which should be taken seriously.
