摘要
目的探讨肺部感染病情对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)并发呼吸衰竭患者无创通气效果的影响。方法选取2015年6月-2017年10月医院收治的82例肺部感染的COPD患者为研究对象,对所有患者进行肺部感染评分(CPIS),根据评分结果分为研究组(CPIS≤7分)39例与对照组(CPIS>7分)43例,所有患者给予无创通气,对比分析两组通气治疗效果。结果无创通气治疗2h后,研究组患者HR、RR、MAP均低于对照组患者,PaO2高于对照组患者(P<0.05);研究组患者总有效率为97.44%,高于对照组患者总有效率67.44%,两组患者疗效比较差异有统计学意义(P<0.05);两组患者通气过程中断次数及耐受性比较差异无统计学意义。结论肺部感染程度是影响COPD合并呼吸衰竭患者无创通气疗效的因素之一,CPIS≤7分的COPD合并呼吸衰竭患者应用无创通气疗效较好。
OBJECTIVE To explore the impact of pulmonary infection on non-invasive ventilation of chronic obstructive pulmonary diseases（COPD）patients complicated with respiratory failure.METHODS A total of 82 COPD patients complicated with pulmonary infection who were treated in the hospital from Jun 2015 to Oct 2017 were recruited as the study objects and divided into the study group with 39 cases（CPIS no more than 7 points）and the control group with 43 cases（CPIS more than 7 points）according to the clinical pulmonary infection score（CPIS）,all of the patients were treated with non-invasive ventilation,and the curative effects of the ventilation were observed and compared between the two groups of patients.RESULTS The HR,RR and MAP of the study group were significantly lower than those of the control group after the non-invasive ventilation for 2 hours,the PaO2 of the study group was significantly higher than that of the control group（P〈0.05）.The total effective rate of the study group was 97.44%,significantly higher than 67.44% of the control group（P〈0.05）.There was no significant difference in the number of times of interruption of ventilation or tolerance to the ventilation between the two groups of patients.CONCLUSIONS The severity of pulmonary infection may be one of the factors leading to the failure of non-invasive ventilation of the COPD patients complicated with respiratory failure,and the non-invasive ventilation can achieve remarkable curative effect on the COPD patients complicated with respiratory failure whose CPIS is no more than 7 points.
