摘要
目的探讨多发性骨髓瘤(MM)患者血清微小RNA-148a(miR-148a)的表达及临床意义。方法采用实时荧光定量PCR法检测50例初诊MM患者(MM组)和30例健康体检者(C组)血清miR-148a的表达。分析其与临床指标的相关性,评估血清miR-148a对MM诊断和预后的预测价值。结果 MM组血清miR-148a表达高于C组(P<0.05)。血清miR-148a诊断MM的ROC曲线下面积为0.746[95%CI(0.635~0.857),P<0.01];取0.791为诊断界值时,其诊断MM的灵敏度为76.0%,特异度为70.0%。以血清miR-148a和β2微球蛋白联合预测因子0.613为诊断界值时,诊断MM的灵敏度为72.0%,特异度为86.7%。MM组miR-148a高表达患者(>0.801)总生存期低于miR-148a低表达患者(≤0.801)(P<0.05)。Cox回归分析显示,血清miR-148a为MM预后的独立影响因素(P<0.05)。MM组血清miR-148a表达与乳酸脱氢酶和免疫球蛋白A(IgA)+IgG+IgM浓度均呈正相关(rs=0.289和0.319,P<0.05)。结论检测血清miR-148a可能为临床上MM的诊断和预后评估等提供实验室依据。
Objective To investigate the expression and clinical significance of microRNA-148 a(miR-148 a)in the patients with multiple myeloma(MM).Methods The expression of serum miR-148 a was detected by real-time fluorescent quantitative PCR in 50 newly diagnosed MM patients(group A)and 30 healthy people(group B).The correlation of serum miR-148 aand clinical indicators were analyzed.The values of serum miR-148 ain the diagnosis of MM and prediction of prognosis for MM patients were evaluated.Results The expression of serum miR-148 ain group A was higher than that in group B(P〈0.05).The area under the ROC curve of serum miR-148 ain diagnosing MM was0.746[95% CI(0.635-0.857),P〈0.01].Taking serum miR-148 a0.791 as the best cut-off value,the sensitivity and specificity in the diagnosis of MM were 76.0% and 70.0%,respectively.Taking combining predictive factor 0.613 as the best cut-off value,the sensitivity and specificity in the diagnosis of MM were 72.0% and 86.7%,respectively.The overall survival of the patients with high expression of miR-148 a(〉0.801)was lower than that in those with low expression of miR-148 a(≤0.801)in group A(P〈0.05).Cox regression analysis showed that serum miR-148 a was an independent factor affecting the prognosis of MM patients(P〈0.05).The expression of serum miR-148 ain group A was positively correlated with the concentrations of lactate dehydrogenase and immunoglobulin A(IgA)+IgG +IgM(rs=0.289 and 0.319,P〈0.05).Conclusion Detection of serum miR-148 amay provide a laboratory basis for clinical diagnosis and prognosis evaluation of MM.
