摘要
目的探讨肺泡灌洗液SAT-TB联合结核菌Xpert检测在诊断涂阴肺结核中的应用价值。方法选择68例涂阴肺结核患者为研究对象,取肺泡灌洗液分别进行SAT-TB、结核菌Xpert检测,比较这两种检查方法单独检测和联合检测的结果。结果在灌洗液结核菌培养阳性患者中,23例SAT-TB阳性、3例SAT-TB阴性,21例结核菌Xpert检测阳性、5例结核菌Xpert检测阴性。在灌洗液结核菌培养阴性患者中,13例SAT-TB阳性、29例SAT-TB阴性,20例结核菌Xpert检测阳性、22例结核菌Xpert检测阴性。SAT-TB、结核菌Xpert检测诊断的灵敏度分别为88.46%、80.77%,特异度分别为69.05%、52.38%,准确性分别为76.47%、63.23%;联合检测诊断的灵敏度为92.30%,特异度为78.57%,准确性为83.82%。联合检测诊断涂阴肺结核的灵敏度、特异度及准确性较单独检测均有显著提高(P<0.05)。结论肺泡灌洗液SAT-TB联合结核菌Xpert检测对诊断涂阴肺结核有一定应用价值,是一种简便、快速、高效的检查方法。
Objective To investigate the value of combined bronchoalveolar lavage fluid SAT-TB and mycobacterium tuberculosis Xpert assay in the diagnosis of smear-negative pulmonary tuberculosis. Methods Bronchoalveolar lavage fluid was collected from 68 patients with smear-negative pulmonary tuberculosis to perform SAT-TB and tubercle bacillus Xpert detection.The results of SAT-TB,tubercle bacillus Xpert and their combination were compared. Results Among the lavage fluid tuberculosis culture-positive patients,23 were SAT-TB-positive,3 were SAT-TB-negative,21 were Xpert-positive,and 5 were Xpert-negative.Of the lavage fluid tuberculosis culture-negative patients,13 were SAT-TB-positive,29 were SAT-TB-negative,20 were Xpert-positive,and 22 were Xpert-negative.The sensitivity,specificity and accuracy of combined detection（92.30%,78.57% and 83.82%,respectively） were significantly higher than those of SAT-TB determination（88.46%,69.05% and 76.47%,respectively） or Xpert assay（80.77%,52.38% and 63.23%,respectively） alone（ P 〈0.05）. Conclusion As a simple,rapid and effective detection method,combined bronchoalveolar lavage fluid SAT-TB and mycobacterium tuberculosis Xpert assay is of great value in the diagnosis of smear-negative pulmonary tuberculosis.
