逍遥解郁方对卒中后抑郁患者神经功能及单胺类神经递质分泌的影响 被引量:11

Effect of Xiaoyao Jieyu Decoction on Neurological Function and Secretion of Monoamine Neurotransmitters for Patients with Post-stroke Depression

摘要 目的:研究逍遥解郁方对卒中后抑郁患者神经功能及单胺类神经递质表达水平的影响。方法:选取自2016年12月~2018年6月本院接诊的92例脑卒中后抑郁患者为研究对象,将患者随机分为对照组和研究组。对照组44例患者,服用盐酸氟西汀胶囊治疗,研究组48例患者,在对照组基础上加服逍遥解郁方。详细记录患者氧化应激指标、炎症因子、神经损伤标志物和单胺类神经递质等指标表达水平。结果:治疗后,两组患者血清神经损伤标志物较治疗前脑源性神经营养因子(BDNF)水平升高,S100B蛋白(S100B)和神经元特异性烯醇化酶(NSE)水平降低,且研究组患者S100B和NSE水平低于对照组患者,BDNF水平高于对照组患者,差异有统计学意义(P<0.05);治疗后,两组患者血清单胺类神经递质多巴胺(DA)、5-羟色胺(5-TH)和去甲肾上腺素(NE)表达水平较治疗前均升高,且研究组患者DA、5-TH和NE表达水平高于对照组患者,差异有统计学意义(P<0.05);治疗后,两组患者血清炎症因子肿瘤坏死因子α(TNF-α)、白介素-1(IL-1)、白介素-2(IL-2)和白介素-23(IL-23)表达水平较治疗前均降低,且研究组患者TNF-α、IL-1、IL-2和IL-23表达水平低于对照组患者,差异有统计学意义(P<0.05);治疗后,两组患者血清氧化应激指标丙二醛(MDA)和脂质过氧化氢(LHP)表达水平较治疗前降低,谷胱甘肽过氧化物酶(GSH-Px)和过氧化氢酶(CAT)表达水平较治疗前升高,且研究组患者MDA和LHP表达水平低于对照组患者,GSH-Px和CAT表达水平高于对照组患者,差异有统计学意义(P<0.05)。结论:逍遥解郁方能够有效的增加患者神经递质分泌,优化患者神经功能,降低患者炎症反应,疗效显著。 Objective:To study the effect of Xiaoyao Jieyu Decoction on neurological function and the expression of monoamine neurotransmitters for patients with post-stroke depression.Methods:Selected 92 patients with post-stroke depression treated in our hospital from December 2016 to June 2018 were randomly divided into controlled group and study group,44 patients in the controlled group,mainly taking Flumethrin Hydrochloride Capsule,the study group of 48 patients,on the basis of the controlled group plus Xiaoyao Jieyu Recipe.The level of expression of indicators such as oxidative stress indicators,inflammatory factors,markers of nerve injury,and monoamine neurotransmitters was recorded in detail.Results:After treatment,serum neurotoxicity markers in the two groups were higher than before treatment,brain-derived neurotrophic factor(BDNF)levels,S100 B protein(S100 B)and neuron-specific enolase(NSE)levels were decreased,and the study group patients S100 B and NSE levels were lower than those in the controlled group,and BDNF levels were higher than those in the controlled group(P〈0.05).After treatment,serum monoamine neurotransmitters dopamine(DA)and serotonin were found in both groups.The expression levels of(5-TH)and norepinephrine(NE)were higher than before treatment,and the expression levels of DA,5-TH,and NE in the study group were higher than those in the controlled group(P〈0.05).After treatment,serum inflammatory cytokines tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),interleukin-2(IL-2)and interleukin-23(IL-23)expression levels were lower in both groups.Before treatment,the levels of TNF-α,IL-1,IL-2,and IL-23 were lower in the study group than in the controlled group(P〈0.05).After treatment,serum levels of the two groups were reduced with Oxidative Stress Indicators Malondialdehyde(MDA)and Lipid Hydroperoxide(LHP).The levels of glutathione peroxidase(GSH-Px)and catalase(CAT)were higher than before treatment,and the expression levels of MDA and LHP were lower in the study group than in the controlled group.The expression levels of GSH-Px and CAT were significantly higher than those in the controlled group(P〈0.05).Conclusion:Xiaoyao Jieyu Recipe can effectively increase the secretion of neurotransmitters in patients,optimize the neurological function of patients,and reduce the inflammatory response in patients with significant curative effect.
