摘要
股骨粗隆间骨折指股骨颈基底至小粗隆水平之间的骨折,常见于外力导致的骨质疏松性骨折。股骨粗隆间骨折分型众多,依据提出时间,临床运用广泛度以及影像学数据种类,分型差异较大。发表于20世纪的分型主要基于X线数据,目前临床较为常用,包括Evans分型、Jensen分型、Boyd-Griffin分型等。最近5年提出的分型主要基于CT数据,临床使用并不普及,包括Nakano分型、Shoda分型以及Kijima分型等。粗隆间骨折分型对指导内固定治疗以及估计预后方面均有较大的价值,但越来越多的研究提示20世纪左右提出的分型存在局限性,需要进一步修正和验证。近5年提出的新式分型需要在临床上继续接受实践检验。研究股骨粗隆间骨折分型,有利于准确评估患者骨折程度,指导临床内固定治疗以及预测预后,具有重要临床意义。本文就股骨粗隆间骨折分型最新研究进展进行综述。
Femur intertrochanteric fracture is defined as the fracture between femoral neck base and small trochanter level, which is often caused by external force in patients with osteoporosis. There are many classifications of the femoral intertrochanteric fractures. According to the publishing time, extensive universality and imaging data, classifications are significantly different. The classifications, mainly published in twentieth century based on X-ray data and commonly used in clinic practice,include Evans Type, Jensen Type and Boyd-Griffin Type. However, the new classifications, based on CT data and published in the last 5 years, are not widely used in clinical setting, including Nakano Type, Shoda Type, and Kijima Type. Although the old classifications proposed in the last century are still of great value for guiding the treatment with internal fixation and predication of prognosis, increasing studies indicate that these were limited and needed further revision or validation. By contrast, the new classifications need to be tested continuously in clinical practice. In short, the classification of intertrochanteric fractures is helpful to evaluate fracture extent accurately, guide clinical fixation and predict prognosis, which has important clinical significance.Thus, the latest research progress of intertrochanteric fracture classification is reviewed in this paper.
