摘要
子宫内膜异位症是妇科常见的复发率极高的疑难病,其中渐进性痛经是其突出表现之一,根据症状特征归属于中医＂痛经＂范畴。因持久反复痛经而产生的焦虑抑郁等负性心理应激,加速了疾病的进展,严重影响女性生活质量。随着身心医学的发展,子宫内膜异位症痛经患者的精神心理健康成为国内外研究者的关注焦点。现代医学多采取口服药物或手术治疗痛经并针对其精神心理问题加以干预,临床效果肯定,但存在一定局限性。《黄帝内经》云：＂诸痛痒疮皆属于心＂,其中＂痛属于心＂在痛经的辨证诊治中有着广泛的临床指导价值。治疗子宫内膜异位症时中医辨证从整体出发,形神同调,在补肾活血为大法的基础上,注重调心安神,疗效显著。将子宫内膜异位症痛经合并精神心理问题的中西医治疗进展进行综述。
Endometriosis is a frequently-encountered and difficult disease in gynecology. The main sequelae of endometriosis is represented by progressive dysmenorrhea. It belongs to dysmenorrhea in Chinese medicine according to the characteristics of symptoms. Anxiety,depression and other negative psychological stress caused by persistent and recurrent dysmenorrhea accelerate the progress of the disease and seriously affect the life quality of women. With the development of psychosomatic medicine,the mental health of dysmenorrhea patients with endometriosis has attracted many scholars＇ attention. Modern medicine often adopts oral medicine or surgical treatment for dysmenorrhea,and cooperates the methods of intervention when dealing with psychological problems. The clinical effect is certain,but there are some limitations. Huangdi＇s Classic records：＂All kinds diseases with pain,itching and sore are exclusively related to the heart＂,which has a wide range of clinical significance in the treatment of dysmenorrhea. In Chinese medicine,the treatment of EM is based on the fundamental law of tonifying kidney and invigorating the circulation of blood,combined with harmonizing spirit,which reflects the concept of wholism and has a remarkable clinical effect. This article summarizes the progress of treatment in the TCM and Western medicine on EM dysmenorrhea combined with psychological problems.
