吸入乙酰半胱氨酸治疗COPD的疗效及对痰液粘稠度、气道阻力的影响 被引量:32

摘要 目的观察吸入乙酰半胱氨酸对慢性阻塞性肺疾病(COPD)的疗效及对痰液粘稠度、气道阻力的影响。方法利用随机抽签法将我院76例COPD分为两组,各38例。对照组采取COPD常规治疗方法,研究组在其基础上雾化吸入乙酰半胱氨酸,比较两组治疗前后痰液粘稠度、气道阻力、血清炎症因子[白介素13(IL-13)、巨噬细胞炎症蛋白-1α(MIP-1α)、肿瘤坏死因子α(TNF-α)]、肺功能、不良反应及疗效。结果研究组治疗总有效率为94.74%,明显高于对照组的76.32%(P<0.05);治疗2周后,研究组痰液粘稠度明显优于对照组(P<0.05),IL-13、MIP-1α、TNF-α水平、R5、Z5、FRES、RV/TLC均明显低于对照组(P<0.05),FEV1明显高于对照组(P<0.05);两组不良反应总发生率比较无显著差异(P>0.05)。结论吸入乙酰半胱酸治疗COPD,可有效降低痰液粘稠度与气道阻力,减轻机体炎症反应,疗效良好,且不会加重不良反应。 Objective To observe the efficacy of inhaled acetylcysteine against chronic obstructive pulmonary disease( COPD) and its effects on sputum viscosity and airway resistance. Methods Seventy-six cases of patients with COPD treated in our hospital were randomly divided into two groups,with 38 cases in each. The control group was given routine treatment for COPD while the study group was additionally treated with aerosol inhalation of acetylcysteine. The sputum viscosity,airway resistance,serum inflammatory factors [interleukin 13( IL-13),macrophage inflammatory protein-1α( MIP-1α),tumor necrosis factor α( TNF-α) ],pulmonary function,adverse reactions and efficacy were compared between the two groups. Results The total effective rate of treatment in the study group was higher than that in the control group( 94.74% vs 76.32%)( P〈0.05). After two weeks of treatment,the sputum viscosity of the study group was significantly better than that of the control group( P〈0.05),and the levels of IL-13,MIP-1α and TNF-α,R5,Z5,FRES and RV/TLC were significantly lower than those in the control group( P〈0.05),but FEV1 level was significantly higher than that in the control group( P〈0.05). There was no significant difference in the total incidence of adverse reactions between the two groups( P〈0. 05). Conclusion Inhaled acetylcysteine for COPD can effectively reduce the sputum viscosity and airway resistance and alleviate body inflammation without increasing adverse reactions.
