摘要
目的:探讨糖类抗原125(CA125)、腺苷脱氨酶(ADA)、外周血中性粒细胞与淋巴细胞比值(NLR)和血小板与淋巴细胞比值(PLR)在胸腔积液鉴别诊断中的价值。方法:选取我院2015年1月至2016年12月住院确诊恶性胸腔积液(A组)患者58例和结核性渗出性胸膜炎(B组)患者62例,收集两组患者血浆CA125、胸水CA125、胸水ADA及血常规的各项指标。采用SPSS 17.0统计软件对数据进行统计学分析。结果:A组患者血浆CA125、胸水CA125、白细胞计数、中性粒细胞计数均明显高于B组,A组患者胸水ADA明显低于B组,两组差异均有统计学意义(P<0.05)。比较A组和B组患者的单核细胞计数、淋巴细胞计数、血小板计数、NLR及PLR,差异无统计学意义(P>0.05)。结论:胸水ADA和胸水CA125在A组患者和B组患者的鉴别诊断中有重要价值,NLR和PLR对两者鉴别诊断意义不大。
Objective：To discuss the differential diagnosis value of CA125,ADA, NLR and PLR in pleural effu-sion. Methods ：58 patients with malignant pleural effusion（ group A） and 62 patients with tuberculous pleurisy（ group B） were selected from January-2015 to December 2016. Plasma CA125 ,pleural effusion CA125 ,pleural effusion ADA and the indexes of blood routine were collected in the two groups of patients. Results ：The plasma CA125 ,pleural effu-sion CA125, the leukocyte and neutrophil count in group A was significantly higher than that in group B and the differ-enee was statistically significant （ P 〈 0.05 ）. Pleural effusion ADA in group A was significantly lower than that in group B and the difference was statistically significant（ P 〈 0.05 ）. There was no difference in monoeyte, lymphocyte, platelet count,NLR and PLR between group A and group B （ P 〉 0.05 ）. Conclusion ： Pleural effusion ADA, pleural effusion CA125 have great value in the diagnosis of A and B group. NLR and PLR are not of great significance for the differential diagnosis of pleural effusion.
