摘要
目的:观察菖蒲郁金汤联合丙种球蛋白治疗小儿病毒性脑炎的临床疗效,以及对血清神经功能指标和脑脊液中S100-β蛋白、sVCAM-1表达的影响。方法:将130例病毒性脑炎患儿按随机数表法分为观察组和对照组,每组65例。观察组给予菖蒲郁金汤(石菖蒲9 g,郁金6 g,栀子9 g,连翘6 g,牡丹皮9 g,天竺黄6 g,竹叶9 g)联合丙种球蛋白治疗;对照组给予丙种球蛋白治疗。观察两组患者的临床疗效,以及血清神经功能指标及脑脊液中S100-β蛋白、sVCAM-1表达情况。结果:观察组有效率为95.38%,对照组有效率为84.62%,观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者S100-β蛋白、人脑髓鞘碱性蛋白(myelin basic protein,MBP)、神经元特异性烯醇化酶(neuron-specific enolase,NSE)和神经生在因子(nerve growth factor,NGF)含量均有明显的下降,差异均有统计学意义(P<0.05);且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组和对照组脊液中S100-β蛋白、sVCAM-1表达水平均明显下降,差异均有统计学意义(P<0.05);且观察组明显低于对照组,差异有统计学意义(P<0.05)。结论:菖蒲郁金汤联合丙种球蛋白治疗小儿病毒性脑膜炎疗效显著,能够有效缓解患者症状,改善神经功能,降低S100-β蛋白、sVCAM-1表达。
Objective： To observe the clinical efficacy of Zhangpu Yujin Decoction combined with Gamma globulin in treatment of viral encephalitis in children,as well as the effects on serum neurological function and expression of S100-β protein and sVCAM-1 in cerebrospinal fluid. Methods： 130 children with viral encephalitis were divided into observation group and control group according to random number table,with 65 cases in each group. The observation group was given Changpu Yujin Decoction（ Acorus gramineus 9 g,Curcuma aromatica 6 g,Gardenia 9 g,Forsythia 6 g,Mudanpi 9 g,Tabasheer 6 g,bamboo leaf 9 g） combined with Gamma globulin treatment,and the control group was treated with Gamma globulin. The clinical efficacy,serum neurological function and expression of central nervous system specific protein（ S100-β） and sVCAM-1 in cerebrospinal fluid were observed and compared. Results： The effective rate of the observation group was 95. 38%,and the effective rate of the control group was84. 62%. That of the observation group was higher than that of the control group,and the difference was statistically significant（ P〈0. 05）. After treatment,S100-β,human brain,the contents of myelin basic protein（ MBP）,neuron-specific enolase（ NSE）and nerve growth factor（ NGF） of the observation group and the control group were significantly decreased. That of the observation group was significantly lower than that of the control group（ P〈0. 05）. After treatment,the expression levels of S100-β and sVCAM-1 in the spinal fluid of the observation group and the control group were decreased and the difference was statistically significant（ P〈0. 05）. That of the observation group was significantly lower than that of the control group（ P〈0. 05）. Conclusion：Changpu Yujin Decoction combined with Gamma globulin is effective in treatment of viral meningitis in children. It can effectively relieve the symptoms,improve the nerve function and decrease the expression of S100-β and sVCAM-1.
