摘要
目的观察曲安奈德联合复方樟柳碱治疗非动脉炎性前部缺血性视神经病变(NAION)的疗效,为该病的诊疗提供依据。方法将临床确诊的40例NAION患者随机分为两组,其中20例接受球后注射曲安奈德联合复方樟柳碱颞侧浅动脉旁治疗(治疗组),另外20例采用颞侧浅动脉旁注射复方樟柳碱治疗(对照组)。均以14天为1个疗程,观察治疗后患者的视力、眼底改变、视盘OCT及视野等表现。结果治疗组最佳矫正视力优于对照组(P<0.05);治疗组眼底改变的治疗有效率为90.0%,对照组为67.5%,治疗组疗效优于对照组,差异有统计学意义(P<0.05);两组视盘OCT情况比较,I级患者中,治疗组例数多于对照组,差异有统计学意义(P<0.05),两组Ⅱ~Ⅳ级的患者例数比较无显著差异;视野改善情况的比较中,治疗组有效率为85.0%,对照组为62.5%,治疗组疗效优于对照组,差异有统计学意义(P<0.05)。结论曲安奈德联合复方樟柳碱治疗可以快速提高视力,消除急性期NAION患眼的视盘水肿,改善视野。
Objective To observe the curative effect of the non-arterial inflammatory anterior ischemic optic neuropathy（NAION）treated with triamcinolone combined compound anisodine.Methods 40 cases of clinical diagnosed NAION were randomly divided into two groups,20 cases accepted the ball joint compound anisodine injection of triamcinolone acetonide temporal shallow artery injection（treatment group）and another 20 patients with temporal shallow artery by compound anisodine injection treatment（control group）.14 days for a period of treatment,and vision,fundus changes,OCT and field of vision were observed before and after treatment.Results The best corrected visual acuity in treatment group was better than that of control group（P〈0.05）.The effective rate of fundus changes in treatment group being 90%,was better than 67.5% of control group,the difference was statistically significant（P〈0.01）;The comparison of OCT situation in two groups：the patients of I level in treatment group were more than control group,the difference was statistically significant（P〈0.05）.The patients ofⅡ~Ⅳlevel in two groups were not statistically significant.For the vision damage effect comparison,effective rate in treatment group being 85% was better than 62.5% of control group,the difference was statistically significant（P〈0.01）.Conclusion Triamcinolone combined compound anisodine can improve eyesight rapidly,eliminate the optic disc edema of the ophthalmic stage of acute stage,improve the field of vision.
