慢性肾脏病3~5期患者血清25-羟维生素D3水平的影响因素分析 被引量:5

摘要 目的探讨慢性肾脏病(CKD)3~5期患者血清25-羟维生素D3[25(OH)D3]水平的影响因素。方法我院收治的172例CKD3~5期患者,其中113例25(OH)D3缺乏(缺乏组),59例25(OH)D3不缺乏(非缺乏组)。对比两组临床资料,分析影响患者25(OH)D3水平的影响因素。结果缺乏组血红蛋白(Hb)、碱性磷酸酶(ALP)、全段甲状旁腺激素(iPTH)、估算肾小球滤过率(eGFR)水平低于非缺乏组;总胆固醇(TC)、白蛋白(Alb)、收缩压(SBP)水平高于非缺乏组(P<0.05)。多元线性回归分析表明,患者体内ALP、Alb和TC水平是导致发生25(OH)D3缺乏的独立危险因素(P<0.05)。结论CKD 3~5期患者存在25(OH)D3缺乏情况,随着病情发展,25(OH)D3缺乏情况加重,因此可以通过有效改善CKD患者的营养状况、脂质代谢情况和骨代谢紊乱等达到改善患者25(OH)D3缺乏情况的目的。 Objective To investigate the influencing factors on serum 25-hydroxyvitamin D-3[25( OH) D3]in patients with 3-5 stage chronic kidney disease( CKD).Methods A total of 172 patients with stage 3 - 5 stage CKD in our hospital were retrospectively analyzed.Of these patients,113 cases lacked 25( OH) D3( lack group) and 59 patients did not lack 25( OH) D3( non-lacking group).The clinical data of the two groups were compared to analyze the influencing factors on 25( OH) D3.Results Compared to the non-lack group,the levels of hemoglobin( Hb),alkaline phosphatase( ALP),full-parathyroid hormone( iPTH),and estimated glomerular filtration rate( eGFR) in the lack group were lower while the levels of total cholesterol( TC),albumin( Alb) and systolic blood pressure( SBP) were higher( P〈0. 05).Multiple linear regression analysis showed that ALP,Alb and TC levels were independent risk factors for 25( OH) D3 deficiency in these patients( P〈0. 05).Conclusion There is a lack of 25( OH) D3 in 3-5 stage CKD patients.With the progression of the disease,25( OH) D3 deficiency is aggravated. Therefore,we can effectively improve the nutritional status,lipid metabolism and bone metabolic disorders to improve the lack status of 25( OH) D3 in CKD patients.
