摘要
基于"气虚宜掣引之"的理论阐述了"掣引"之法治疗溃疡性结肠炎的思路。"掣引"之法是指在本虚标实的情况下,采取挽回、导引阳气上升,补其气而升其阳的治法,以调畅气机令补而不滞。溃疡性结肠炎病位在肠腑,病因病机复杂,以脾肺肾三脏亏虚为本,肠腑湿瘀互结为标,认为脾失健运、水湿停聚为本病发生的始动因素,肺失治节、气血壅滞、久病及肾、关门不固是该病发展的关键。若单用补益之剂恐闭塞气机、有恋邪之弊,故治宜"掣引"之法。根据张景岳对"掣引"的释义,具体治疗通过温补脾气、益气升阳、提掣肺气、调和气血、摄纳肾气、平衡阴阳的方法掣引脾肺肾之气,以达到扶正祛邪、改善临床症状、促进溃疡愈合的目的。
Elaborated the treatment on ulcerative colitis（ UC） based on the theory of ＂Qi deficiency is suitable for elevating＂. The＂elevating＂method is a treatment of tonifying the Qi and enhances the Yang by using restoring and leading the Yang rise up when the syndrome is root deficiency and branch excess. Move the Qi to make it not to be stagnated. The UC is located in the intestines and the etiology and pathomechanism of disease is complicated. The root of UC is the deficiency of the spleen,lung and kidney,and the tip is dampness and blood stasis jell together in the intestines. It is considered that the spleen losing transformation functions then water retention is the initial factors for the occurrence of the disease,the lung losing governing management and regulation that making Qi and blood stagnation,and kidney Qi insecurity of long-term illness are the key to the disease＇s development. If only tonifying formula is used,fear of blocking Qi and lingering pathogen,therefore,the method of＂elevating＂should be taken. According to ZHANG Jing-yue＇s interpretation of ＂elevating＂to treat through tonifying spleen and warming the Yang Qi,enhancing lungs to reconcile Qi and blood,receiving the kidneys Qi and to make the Yin and Yang be harmony so that could get the goal of reinforcing the health and eliminating the pathogenic,improving the clinical symptoms,and promoting the healing of ulcers.
