摘要
目的:观察加减乌梅丸治疗上热下寒型2型糖尿病的临床效果。方法:选取2015年1月~2016年12月深圳市中医院收治的60例糖尿病患者,采用随机平行对照法分成治疗组及对照组各30例,2组均进行生活方式干预,治疗组采用加减乌梅丸方联合二甲双胍片治疗,对照组采用二甲双胍片治疗;治疗3月后,观察2组治疗前后中医证候积分、空腹血糖(FBG)、餐后2h血糖(2h FBG)、糖化血红蛋白(Hb Alc%),以及血浆胰岛素(FINS、2h INS)、胰岛素抵抗指数(HOMA-IR)和血脂(TC、TG、LDC-C)等指标的变化。结果:治疗后,2组显效率、有效率比较,差异有统计学意义;2组中医证候积分及FBG,2h FBG,Hb Alc%较前明显下降(P<0.05),且治疗组优于对照组,差异均有非常显著性意义(P<0.01);另外研究发现,治疗后,2组FINS、2h INS、HOMA-IR、TC、TG、LDC-C较前显著下降(P<0.05),且治疗组优于对照组,差异均有显著性意义(P<0.05)。结论:加减乌梅丸方能显著改善上热下寒型2型糖尿病患者临床症状,降低患者血糖等指标,另外还能够明显改善胰岛素抵抗,调节脂代谢,临床疗效显著,值得临床推广。
Objective： To observe the effect of Wumei pill in treating cold-heat mixture of type 2 diabetes mel- litus. Methods： From January 2015 to December 2015, 60 patients with diabetes mellitus were enrolled in Shenzhen Chinese Medicine Hospital. 60 cases were divided into treatment group and control group by random parallel control group, 30 cases in each group. All the two groups were treated by lifestyle intervention. The treatment group was treated with addition and subtraction of Wumei pill combined with metformin tablets, the control group was treated with met- formin tablets. After treatment, the levels of TCM syndrome, fasting blood glucose （FBG）, postprandial 2h blood glucose （2hFBG）, glycosylated hemoglobin （HbAlc%）, and plasma insulin （FINS, 2hINS）, insulin resistance index （HOMA -IR） and blood lipid （TC, TG, LDC-C） and other indicators of changes were observed before and after treatment. Results： After treatment, the effective rate was 80%, the total effective rate was 93.3%, the effective rate was 40% in the con- trol group and the total effective rate was 73.3%. After treatment, the scores of TCM syndrome and FBG, 2hFBG and HbAlc were significantly decreased （P〈0.05）, and the treatment group was superior to the control group, the difference was significant （P〈0.05）, and the levels of FINS, 2hINS, HOMA-IR, TC, TG and LDC-C in the two groups were sig- nificantly lower than those in the control group （P〈0.05） The Conclusion： Addition and subtraction of Wumei pill can significantly improve the clinical symptoms of patients with type 2 diabetes mellitus and reduce blood glucose and other indicators, but also can significantly improve insulin resistance, regulate lipid metabolism, clinical efficacy is remark- able, worthy of clinical promotion.
