HR-HPV及其它病原微生物感染对宫颈癌及宫颈上皮细胞内瘤变影响的探讨 被引量:13

摘要 目的探讨高危型人乳头瘤病毒(HR-HPV)及其它病原微生物感染对宫颈癌及宫颈上皮细胞内瘤变(CIN)影响。方法选取经病理活检确诊的宫颈癌患者29例,CINⅠ级84例,CINⅡ级63例,CINⅢ级32例,单纯宫颈炎48例,对照组42例,进行HR-HPV、支原体(CT)、解脲脲原体(Uu)、细菌性阴道病(BV)检测。结果 HR-HPV、CT在宫颈癌组、CIN各级组与炎症组和对照组相比较具有统计学意义P<0.05;Uu、BV在宫颈癌组和CIN各组比较无统计学意义P>0.05,但与炎症组和对照组比较有统计学意义P<0.05。HR-HPV+CT、HR-HPV+Uu、HR-HPV+BV以及HR-HPV合并多重感染在宫颈癌组、CIN各级组间比较均具有统计学意义,P<0.05。且明显呈递减趋势;而HR-HPV单独感染在各组间比较也具有统计学意义P<0.05,但呈明显递增趋势。结论 CT、Uu、BV或其它病原微生物能增加宫颈感染HR-HPV的机会,从而增加了宫颈癌前病变及宫颈癌的发生率。 Objective To explore the influence of high-risk human papilloma virus (HR-HPV) and other pathogenic microorganism infection on cervical cancer and cervical intraepithelial neoplasia (CIN). Methods 29 cases with cervical cancer, 84 cases with CINI,63 cases with CINII,32 cases with CINIII,48 cases with cervicitis and 42 cases with normal cervix were recruited and diagnosed by the pathological biopsy. HR-HPV, Chlamydia trachomatis (CT), Ureaplama urealyticum (Uu), Bacterial vaginosis(BV) were detected in all cases. Results HR-HPV and CT in cervical cancer group and CIN groups were significantly different than those in cervicitis and normal cervix groups with statistical significance(P〈0.05). Uu and BV in cervical cancer group were not sta- tistically significant, compared with those in CIN group (P〉0.05), but obvious significance was observed between those in cervicitis and normal cervix groups (P〈0.05). Additionally,the cases infected HR-HPV combined with CT(HR-HPV+CT),Uu(HR-HPV+U- u), Bv(HR-HPV+BV) and merging uhiple infections were compared in pairs groups with statistical significance(P〈0.05). The infection rate of multiple infections with HR-HPV showed a trend of increase according to cervical cancer and severity of CIN severity. The infection rate of HR-HPV was statistically significant between any two groups(P〈0.05) and also showed the significant increasing trend from CIN to cervical cancer. Conclusion CT, Uu, BV and other pathogenic microorganisms could massively elevate the risk of HR-HPV infection,leading to the increasing incidence of CIN and cervical cancer.
