摘要
目的探讨血清肿瘤标志物神经元特异性烯醇化酶(NSE)、细胞角蛋白-19片段抗原(CYFRA21-1)、癌胚抗原(CEA)、糖类癌抗原199(CA199)、组织多肽特异性抗原(TPS)在肺癌TNM分期中的价值。方法选取2014年1月至2016年12月在大连医科大学附属第二医院住院的肺癌患者,所有患者均检测五种血清肿瘤标志物水平,按照疾病的病理类型、TNM分期进行分组,然后进行相关的统计分析。结果 TPS、CEA、NSE阳性率随着肺癌分期的升高而升高,不同分期之间阳性率有显著性差异(P<0.05);CYFRA21-1、CA199不同分期之间阳性率无显著性差异(P>0.05)。TPS、CEA、NSE在不同分期的水平有显著差异(P<0.05),但CA199、CYFRA21-1水平随疾病分期增加也有升高的趋势,但差异无统计学意义(P>0.05)。血清TPS、CEA、NSE水平鉴别早期和晚期肺癌有统计学意义(P=0.034,0.041,0.016),这三种肿瘤标志物的界值分别是:135.6μg/L、3.36μg/L、13.28μg/L。而血清CYFRA21-1、CA199水平鉴别早期和晚期肺癌无统计学意义(P=0.056,0.072)。结论不同的血清肿瘤标志物在不同TNM分期中的价值不同,准确地利用肿瘤标志物进行TNM分期,可为肺癌TNM分期提供参考。
Objective To investigate the TNM value of serum tumor marker in lung cancer,including neuron-specific enolase （NSE）,cytokeratin fragment 21-1 （CYFRA21-1）,carcinoembryonic antigen （CEA）,carbohydrate antigen 199 （CA199）,tissue polypeptide specific antigen （TPS）.Methods Lung cancer patients who were diagnosed as lung caner in Dalian Medical University from January 2014 to December 2016,met inclusion and exclusion criteria were included into the present study.The NSE,CYFRA21-1,CEA,CA199 and TPS of all patients were detected.The relationship of these serum tumor marker and pathology type,TNM grade was analyzed respectively.Results The positive rate of TPS,CEA and NSE in lung cancer patients increased with the staging increasing.There was significant difference among different stages （P＆lt;0.05）.There was no significant difference in the positive rate of CYFRA21-1 and CA199 among different stages （P＆gt;0.05）.There was significant difference in the TPS,CEA and NSE level among different stages （P＆lt;0.05）.There was the elevatory tendency of CA199 and CYFRA21-1 with the staging increasing.However,no significant difference was found （P＆gt;0.05）.There was significant difference of TPS in different T group （P＆lt;0.05）.There was significant difference of CYFRA21-1 in different N group （P＆lt;0.05）.The NSE,TPS and CEA of M1 group were significantly higher than those of M0 group （P＆lt;0.05）.The cut-off value of the three serum tumor markers was 135.6 μg/L,3.36 μg/L,13.28 μg/L,respectively.There was negative correlation between TPS and KPS,with significant difference （P＆lt;0.05）.Conclusion The value of different serum tumor marker was different in different TNM stage.Combined application of these marker can enhance the accuracy of TNM staging,and can act as a reference factor in the TNM staging.
