摘要
目的研究右美托咪定、曲马多对椎管内麻醉后寒颤的预防作用。方法选择我院2014年8月至2016年8月期间收治的行椎管内麻醉的下肢骨折手术患者80例作为研究对象,均分为两组:右美托咪定组于术前注射生理盐水联合右美托咪定,曲马多组于术前注射生理盐水联合曲马多。观察两组患者的血流动力学情况、寒颤发生情况以及不良反应情况。结果两组各时间点的HR、MAP以及Sp O2水平均无统计学差异(P>0.05)。两组患者治疗前的寒颤发生率与治疗后的寒颤程度均无统计学差异(P>0.05)。右美托咪定组的不良反应发生率为7.5%,显著低于曲马多组的25.0%,差异有统计学意义(P<0.05)。结论右美托咪定和曲马多均可有效降低患者椎管内麻醉后寒颤的发生率,且对血流动力学影响较小,但右美托咪定的不良反应相对更少。
Objective To study the effects of dexmedetomidine and tramadol in prevention of shivering after spinal anesthesia.Methods 80 cases of patients with lower limb fracture surgery under spinal anesthesia in our hospital from August 2014 to August 2016 were selected as research objects and divided into two groups equally. The dexmedetomidine group was injected with physiological saline combined with dexmedetomidine before surgery,while the tramadol group was injected with physiological saline combined with tramadol before surgery. The hemodynamic conditions,incidences of shivering and adverse reactions of two groups were observed. Results No statistical difference was found between two groups in the HR,MAP and SpO2 levels at different time points(P〉0.05). The incidence of shivering before treatment and the degree of shivering after treatment showed no statistical difference between two groups(P〉0.05). The incidence of adverse reactions of dexmedetomidine group was 7.5%,significantly lower than 25.0% of tramadol group,with statistical difference(P〈0.05). Conclusions Both dexmedetomidine and tramadol can effectively reduce the incidence of shivering after spinal anesthesia,and have less impact on hemodynamics,while dexmedetomidine has fewer adverse reactions relatively.
