摘要
目的探讨肺浸润性腺癌的能谱CT碘基物质分析与Ki-67表达程度的相关性。方法回顾性分析经病理证实的44例肺浸润性腺癌患者资料,所有患者的免疫组织化学指标均包括Ki-67,并且于术前行能谱CT增强扫描,包括动脉期及静脉期,分别延迟25 s、55 s。根据CT纵隔窗上肿瘤含实性成分的多少,分成实性结节组(18例)、m GGN组(19例)、p GGN组(7例)。采用Spearman相关法分析所有病例中Ki-67与动脉期及静脉期肿瘤平均碘基值的相关性,及与肿瘤实性成分多少的相关性;分别分析实性结节组、m GGN组、p GGN组中Ki-67与动脉期及静脉期肿瘤平均碘基值的相关性。结果所有病例中,Ki-67与动脉期及静脉期肿瘤平均碘基值均呈负相关性(P<0.05)。另外,Ki-67与肺癌结节中实性成分呈正相关(P=0.02,r=0.36)。实性结节组和m GGN组中,Ki-67与动脉期及静脉期肿瘤平均碘基值均呈负相关性(P<0.05)。p GGN组中,Ki-67与动脉期及静脉期肿瘤平均碘基值均无相关性(P>0.05)。Ki-67分组中动脉期及静脉期平均碘基值均有明显差异性(P<0.05)。结论肺浸润性腺癌的碘基值与Ki-67具有较好的负相关性,在评估肿瘤增殖能力上可以对Ki-67进行补充。
Objective To study the correlation of iodine content measurement and Ki-67 in invasive lung adenocarcinoma. Methods 44 patients with invasive lung adenocarcinoma confirmed by pathology were enrolled in this study. All patients underwent enhancement scan( including arterial phase and venous phase) with spectral CT before operation and acquired immunohistochemical indexes( including Ki-67) after operation. All patients were classified into three groups according to the solid content of tumors on the mediastinal window images,including solid nodules group,m GGN group and p GGN group. The Spearman correlation method was used to analyze the correlation between Ki-67 and iodine content values of tumors in arterial and venous phase for all patients,and to analyze the correlation between Ki-67 and solid content of tumors. In addition,to analyze the correlation between Ki-67 and iodine content values of tumors in arterial and venous phase for solid nodules group,m GGN group and p GGN group. Results In 44 patients,the numbers of solid nodules group,m GGN group and p GGN group were 18,19 and 7 cases,respectively. There was negative correlation between Ki-67 and iodine content values of tumors in arterial and venous phase for all patients,solid nodules group and m GGN group( P 〈0. 05),except p GGN group( P 〉 0. 05). On the other hand,there was positive correlation between Ki-67 and solid content of tumors( P = 0. 02,r = 0. 36). Conclusion There was good negative correlation between iodine content values and Ki-67 in invasive lung adenocarcinoma,which can be helpful for evaluating tumor proliferation capacity by Ki-67.
