慢性乙醇中毒性周围神经病的脑部电生理学研究 被引量:1

摘要 目的分析慢性乙醇中毒性周围神经病(CAPN)患者的脑功能损害。方法以CAPN患者184例为研究组,同期健康志愿者169例为对照组。应用神经电生理学技术,分析脑听觉诱发电位(BAEP)、事件相关电位;应用脑电监测非线性分析技术,分析脑电信号的关联维及近似熵。结果①BAEP:CAPN组在波Ⅰ~Ⅶ的潜伏期均显著性延长(t=7.290~35.217,P<0.01),波幅均显著性降低(t=5.567~18.389,P<0.01)。②ERPs:CAPN组在事件相关电位各成分的潜伏期均显著性延长(t=9.086~29.824,P<0.01),波幅均显著性降低(t=20.139~37.989,P<0.01)。③脑区ApEn:CAPN组在额区、颞区、顶区、枕区的脑电信号近似熵均显著性偏低(t=18.318~33.905,P<0.01)。④全脑D_2及ADEn:CAPN组全脑关联维(t=5.826,P=0.0001)、近似熵(t=2.961,P=0.0033)均显著性偏低。结论 CAPN患者在中枢神经系统临床表现出现前,已经存在脑皮层及皮层下功能减退,脑电生理学技术有助于其早期诊断。 Objective To analyze the brain functional lesion of sufferers with chronic alcoholism peripheral neuropa- thy(CAPN).Methods 184 sufferers with CAPN were taken as research group,while 169 healthy volunteers were tak- en as control group.Electroneurophysiologic technique was adopted to analyze brain auditory evoked potential(BAEP) and event related potential (ERP);EEG monitoring nonlinear analysis technique was adopted to analyze the correlation dimension(D2) and approximate entropy(ApEn) of electroencephalogram.Results ①The latency stage of BAEP in wave I - WlI in the research group was significantly prolonged(t= 7.290 - 35.217, P〈 0.01 ), and the amplitude in the research group was decreased significantly(t= 5.567 - 18.389, P〈 0.01).②The latency stage of event related potential (ERP) components the research group was significantly prolonged (t= 9.086-29.824, P〈 0.01), and the amplitude in the research group was decreased significantly(t = 20.139 - 37.989, P〈 0.01). ③Brain ApEn of frontal lobe, temporal lobe, parietal lobe, and occipital lobe was significantly low in the research group (t = 18.318 - 33.905, P 〈 0.01 ). ④The whole brain correlation dimension (D2) (t = 5.826, P = 0.000 1) and approximate entropy (ApEn) (t = 2.961, P = 0.003 3) in the research group was significantly low.Conclusion Before the clinical manifestations of central nervous system in CAPN patients ,there have been the existences of brain cognitive dysfunction.The nerve electrophysiological techniques will be used in early diagnosis efficiently.
