黑质“燕尾征”消失和黑质体积缩小在帕金森病诊断价值的临床分析 被引量:1

Application of swallow tail appearance and volume measurement of substantia nigra in advanced patients with Parkinson's disease

摘要 目的探讨黑质"燕尾征"消失和黑质体积缩小在帕金森病患者影像学诊断的应用价值。方法选取2016年2月至2016年8月在我院神经外科行STN-DBS手术治疗的24例帕金森病人作为观察组,并选择36例健康志愿者作为对照组,所有患者行3.0T MR磁敏感加权成像扫描,统计两组黑质"燕尾征"消失率和黑质标准化体积改变的情况,分析两组间的差异。结果观察组中黑质"燕尾征"消失率和标准化体积分别为100%和1.32±0.23cm^3,对照组中分别为1.38%和1.49±0.18cm^3,两者相比,观察组燕尾征消失率显著高于对照组,并且黑质体积显著小于对照组,差异有统计学意义(P<0.01)。结论帕金森病患者普遍存在黑质"燕尾征"消失和黑质体积缩小,可以作为一种影像学诊断指标。 Objective To evaluate the clinical value of swallow tail appearance in detecting and volume reduction of Nigro the patients with Parkinson's disease at 3.0T MR SWI. Methods A total of 24 patients with STN-DBS surgery and 36 age-matched normal controls were scanned by using 3.0T susceptibility-weighted(SWI). Substantia Nigra (SN) was selected as region of interest (ROD and observed the swallow tail appearance and measured the volume on SWI. X^2 test and t test were used for comparison between PD patients groups and normal controls. There was statistically difference if the P value was lower than 0.05. Results Observation group SN swallow tail disappearance rate and the standardized volume were 100% and 1.32 ± 0. 23 cm^3 ,while the control group was 1.38% and 1.49± 0.18 cm^3. In comparison,the observation group swallow tail disappearance rate is significantly higher than the control group, and the SN volume significantly less than the control group,the difference was statistically significant(P 〈0.01) . Conclusion Swallow tail disappearance and volume reduction are universal in the sub- stantia nigra of Parkinson's disease people in the magnitude imaging at 3.0T MR SWI;It can be used as an imaging diagnostic indicators.
